  • Diagnostic and therapeutic strategy for postpancreaticoduodenectomy hemorrhage–experience of a single center

Objective To investigate the cause, treatment, and prognosis of the postpancreaticoduodenectomy hemorrhage (PPH) . Method A total of 779 patients who underwent pancreaticoduodenectomy at Fudan University Shanghai Cancer Center between January 2015 and December 2016 were enrolled, and the data of them were retrospectively analyzed. Results Sixteen patients (PPH group) suffered from PPH and 763 patients didn’t suffered from PPH (non-PPH group) of 779 patients. There was no significant difference in the age, gender, type of disease, operative time, blood loss, and ratio of blood transfusion between the 2 groups (P>0.05), but the incidences of pancreatic fistula and delayed gastric emptying, postoperative drainage time, hospital stay, and mortality were all higher or longer in PPH group compared with non-PPH group (P<0.001). Of the 16 PPH patients, early haemorrhage occurred in 3 patients (including 2 patients with gastrointestinal haemorrhage and 1 patient with intra-abdominal haemorrhage) and delayed haemorrhage occurred in 13 patients (including 6 patients with gastrointestinal haemorrhage and 7 patients with intra-abdominal haemorrhage). All the 3 patients with early haemorrhage were cured by surgery. Of the 13 patients with delayed haemorrhage, 5 patients were cured by radiological intervention, 2 patients were cured by endoscopic hemostasis, and 3 patients were cured by conservative treatment, but 3 patients with sentinel bleeding died after interventional embolization+surgery. Conclusions The mortality of PPH is high. Surgery is optimal to early haemorrhage and radiological intervention is optimal to delayed haemorrhage, including embolization or covered stent implantation. The sentinel bleeding should be given great attention.

  • Treatment of acute pancreatitis based on the integrative theory of typhoid fever, miscellaneous diseases and epidemic febrile diseases

Acute pancreatitis (AP), characterized by complex complications, substantial treatment expenses, and elevated mortality rates in severe instances involving multi-organ dysfunction and post-recovery challenges, has traditionally been addressed through Traditional Chinese Medicine (TCM) based on the “Hot Disease” theory. Recent research, however, indicates potential adverse effects from over-relying on this approach with bitter and cold purgative herbals. Evolving from this, a novel methodology, conceptualized by Professor ZHANG Zailiang, integrates the theory of typhoid fever, miscellaneous diseases and epidemic febrile diseases. This paradigm shift encompasses the entire AP spectrum, emphasizing early interventions for organ damage, proactive complication management, and meticulous post-recovery care. It proposes a comprehensive, tailored strategy for monitoring disease evolution and convalescence, signaling a significant advancement in the AP treatment paradigm, particularly in the context of TCM’s role in critical care.

  • Penicilliosis marneffei Involving the Lung in Healthy Hosts: Three Cases Report and Literature Review

ObjectiveTo explore the clinical characters, diagnosis and treatment of Penicilliosis marneffei (PSM) in healthy hosts. MethodsThe clinical data of three cases of PSM involving the lung, bronchus and pleura in healthy hosts were retrospectively analyzed, and the case reports of Penicillium marneffei (PM) infection in healthy hosts were summarized by searching database for related articles, with "Penicilliosis marneffei" as key word both in English and Chinese literatures. ResultsThe main clinical manifestations of three PSM patients included fever, cough, lymphadenopathy, and elevated white blood cell counts.Series of chest computed tomography of three cases demonstrated atypical and extensive lung lesions. The fiberoptic bronchoscopy of the 2nd case showed several irregular nodules on the bronchial wall. The thoracoscopy of the 3rd case showed much pleural adhesions and small nodules on visceral and parietal pleura.Final diagnosis of PSM was confirmed by the fungal culture, and all patients received antifungal treatment. The symptoms relieved on medication but reoccurred on discontinuation.Literature review identified 36 cases of PSM in healthy hosts. The main clinical features were fever, cough/expectoration, skin rash/skin abscess and elevated white blood cell counts. The most common used antifungal drug was amphotericin B alone or combined with traconazole. The thoracic radiological imaging showed that roughly half of them had clearly thoracic lesions (20/36, 55.65%), and the pulmonary inflammatory infiltrate shadow was most common.Seventeen cases (47.22%) were misdiagnosed as tuberculosis. ConclusionsPulmonary involvement is common in healthy hosts with PSM and the thoracic radiological features are variable and unspecific, so PSM is easy to be misdiagnosed as tuberculosis. The PSM patients need a longer treatment than other fungal infections.

  • Diagnosis and treatment pathway of neoadjuvant immunotherapy for esophageal cancer in Henan province

Esophageal cancer is a highly prevalent tumor species in Henan province, which brings heavy medical burden to families and society. Surgical treatment plays a dominant role in the treatment of non-advanced esophageal cancer. However, cancer cells in esophageal cancer lesions are highly invasive, postoperative recurrence and metastasis rates are pretty high. More effective systemic and comprehensive treatment is urgently needed to improve the prognosis. We invited 52 doctors in esophageal surgery, oncology, pathology, imaging, and radiation therapy of 32 hospitals at all levels in Henan province, to repeatedly negotiate and fully discuss in combination with evidence and clinical practice experience. Finally, “diagnosis and treatment pathway of neoadjuvant immunotherapy for esophageal cancer in Henan province” was formulated. In this treatment pathway, seven recommendations were proposed from seven perspectives including target population, patient evaluation, protocol selection, surgical timing, postoperative management, organ preservation, and general principles to offer reference for medical personnel related to esophageal cancer surgery.

  • Development and application of a digital platform for standardized quality control of thyroid cancer diagnosis and treatment

ObjectiveExplore the impact of a digital-intelligence-based quality control platform for thyroid cancer on the effectiveness of clinical diagnosis and treatment quality management. MethodsThe digital-intelligence-based quality control platform for thyroid cancer at Zhejiang Provincial People’s Hospital was launched at the end of July 2022. In its initial phase, six quality control indicators related to the standardized diagnosis and treatment of thyroid cancer were deployed. This study analyzed the changes in these six quality control indicators between January 2022 and November 2023, comparing data before and after the platform’s implementation. ResultsCompared with the period from January to July 2022 (prior to the platform’s launch), the rates of preoperative cytopathological examination (t=–8.490, P<0.001) and postoperative pTNM staging for thyroid cancer patients (t=–3.027, P=0.013) increased from July to November 2023 (one year post-launch). However, the proportion of minimally invasive surgeries among thyroid cancer patients (t=4.085, P=0.002) decreased. The linear regression model results indicated that, following the platform’s launch, there was a gradual increase over time in both the preoperative cytopathological examination rate for thyroid cancer (P=0.001) and the postoperative pTNM staging rate (P=0.049). ConclusionPreliminary results of this study suggest that the thyroid cancer digital-intelligence-based quality control platform developed by our team can effectively enhance the standardized quality control of clinical diagnosis and treatment for thyroid cancer.

  • Analysis of the current service volume and related factors of primary health care institutions in Chengdu

Objective To investigate the influencing factors in the service volume of primary health care institutions in Chengdu, and provide references for promoting the hierarchical diagnosis and treatment. Methods A self-administered questionnaire was used to investigate the service volume of a total of 390 primary health care institutions (including community health service centers and township hospitals) across 22 districts and counties of Chengdu from October to November 2016. Rank sum test, rank correlation analysis and multiple linear regression model were used to analyze the influencing factors of outpatient visits in the institutions. Results The median annual amount of outpatient visits was 60 493 in community health service centers and 31 374 in township hospitals. Between community health service centers and township hospitals, the difference in median daily visits per doctor was not significant (14.41 vs. 13.29), but the median daily visits per doctor in general practitioners (12.22 vs. 8.16), rehabilitation physicians (10.10 vs. 6.34) and traditional Chinese medical doctors (4.82 vs. 6.17) varied considerably. Multiple linear regression analysis showed that the amount of outpatient visits in community health service centers was related to the amount of intermediate physicians, while the amount of outpatient visits in township hospitals was related to the setup of dental clinics, the amount of primary physicians, the amount of beds and population to be served (P<0.05). Conclusions Differences exist in the service volume among primary health care institutions in Chengdu. It’s necessary to make proper plans for the management of administrative offices and the allocation of personnel, so as to improve medical services of primary health care institutions.
