  • COMPARISON OF FEMORAL CONDYLAR TWIST ANGLE IN THREE DIMENSIONAL RECONSTRUCTION DIGITAL MODELS OF KNEE JOINT BASED ON TWO DIMENSIONAL IMAGES OF MRI AND CT

    ObjectiveTo study the difference of femoral condylar twist angle (CTA) measurement in three dimensional (3-D) reconstruction digital models of human knee joint based on the two dimensional (2-D) images of MRI and CT so as to provide a reference for selecting the best method of CTA measurement in preoperative design for the femoral prosthesis rotational position. MethodsThe CTA of 10 human cadaveric knee joint was measured in 3-D digital models based on MRI (group A), in 3-D digital models based on CT (group B), in the cadaveric knee joint with cartilage (group C), and in the cadaveric knee joint without cartilage (group D), respectively. The statistical analysis of the differences was made among the measurements of the CTA. ResultsThe CTA values measured in 3-D digital models were (6.43±0.53)° in group A and (3.31±1.07)° in group B, showing significant difference (t=10.235, P=0.000). The CTA values measured in the cadaveric knee joint were (5.21±1.28)° in group C and (3.33±1.12)° in group D, showing significant difference (t=5.770, P=0.000). There was significant difference in the CTA values between group B and group C (t=5.779, P=0.000), but no significant difference was found between group A and group C (t=3.219, P=0.110). ConclusionThe CTA values measured in the 3-D digital models based on MRI are closer to the actual values measured in the knee joint with cartilage, and benefit for preoperative plan.

  • REVISION SURGERY FOR CUSTOM-MADE TUMOR PROSTHESIS OF KNEE JOINT

    Objective To evaluate the surgical skill, cause of revision, compl ications, prosthetic survival and postoperative function in revision of custom-made tumor prosthesis replacement of knee joint. Methods The cl inical data of 33 patients who received prosthetic revision surgery between June 2002 and June 2007 were reviewed. There were 17 malesand 16 females with an average age of 33.1 years (range, 16-67 years). The pathological diagnosis included 17 osteosarcomas, 11 giant cell tumors, 2 mal ignant fibrous histocytomas, 1 chondrosarcoma, 1 synovial sarcoma, and 1 l iposarcoma. The involved locations were distal femur in 22 cases and proximal tibia in 11 cases. The average interval between first prosthetic replacement and revision surgery was 45.3 months (range, 6-180 months). The reason for revision included local recurrence in 2 cases, deep infection in 8 cases, aseptic loosening in 7 cases, peri prosthetic fracture in 1 case, prosthetic stem fracture in 6 cases, and prosthetic hinge failure in 9 cases. Six patients with deep infection received two-stage revision surgery, while the other 27 patients received one-stage revision. Cemented prostheses were used in all patients. Allograft prosthetic composite and revisions were used in 2 patients who had deficit of diaphysis for stem fixation. Results In 17 patients who received both primary prosthetic replacement and revision, the operative time was (149.8 ± 40.5) minutes and (189.9 ± 43.8) minutes, and the blood loss was (605.2 ± 308.0) mL and (834.1 ± 429.9) mL for primary prosthetic replacement and revision, respectively; all showing statistically significant differences (P lt; 0.05). The mean time of follow-up was 45.1 months (range, 12-76 months). Heal ing between allograft and host bone was obtained in 2 patients with allograft prosthetic composite and revision after 1.5 years and 2 years, respectively. After revision surgery, 3 patients died of lung metastasis after 12-24 months, and other 3 patients havinglung metastasis were al ive with disease. Nine (30%) compl ications occurred in 30 patients who were al ive at last follow-up. The compl ications included wound infection in 2 patients, deep infection in 5 patients, mechanical problems in 2 patients. Prosthetic failure occurred in 7 patients (23.3%). The 5-year survival rate of revised prosthesis was 68.6%. The Musculoskeletal Tumor Society (MSTS) score at 6 months after revision (73.6% ± 14.4%) was significantly improved (P lt; 0.01) when compared with before revision (57.1% ± 10.6%). Conclusion The main reasons for revision of custom-made tumor prosthesis of knee joint were mechanical problems and deep infection. Although revision surgery of knee is relatively compl icated and has some compl ications, a functional l imb could be maintained in most tumor patients.

  • ANALYSIS OF SHORT-TERM RESULTS OF POST TOTAL KNEE ARTHROPLASTY USING TC-DYNAMIC POSTERIOR STABILIZED PROSTHESIS

    Objective To assess the feasibility, safety, and validity of the TC-Dynamic posterior stabilized prosthesis implanted in the total knee arthroplasty (TKA). Methods Twelve knees of 10 patients (the TC-Dynamic group) were followed up, who had been implanted with the TC-Dynamic posterior stabilized prosthesis from September 2003 to March 2004. Preoperative KSS knee scores were 16.08±11.58, function scores 13.75±19.79, and the range of motion (ROM) of the knee 75.00±26.46°. Meanwhile, 50 knees of 30 patients (the Scorpio group) werefollowed up, who had undergone TKA with the Scorpio posterior stabilized prosthesis.Preoperative KSS knee scores were 19.48±967, function scores 3.16±19.82,andthe ROM of the knee 80.80±22.82°. The anteroposterior and lateral X-ray filmsof each knee were examined before and after operation. The statistical Z-test was used to analyze the differences between the 2 groups in the improvement of the KSS knee scores, function scores, and ROM after operation. Results The average of the 130 days’ follow-up revealed that the patients implanted with the TC-Dynamic prosthesis had an excellent result. In the TC-Dynamic group, the KSS knee scores were 88.83±4.04 with improved scores of 72.75±14.47 compared with those before operation; function scores were 79.17±5.15 with improved scores of 65.42±19.47; the ROM of the knee was 107.92±11.57° with increased degrees of 32.92±32.22°.Meanwhile, in the Scorpio group, the KSS knee scores were 85.68±7.36 with improved scores of 66.20±10.44 compared with those before operation; function scores were 71.40±12.70 with improved scores of 68.24±25.35; the ROM of the knee was 109.20±11.13° withincreaseddegrees of 28.40±26.41°.There was no significant difference in the improvement of the KSS knee scores, function scores, and ROM after operation between the 2 groups (Pgt;0.01). All the X-ray films of the knees implanted with both the Scorpio prosthesis and the TC-Dynamic prosthesis were analyzed.No malalignment or lucent line with the prosthesis was seen in all these X-ray films. Conclusion The short-term follow-up indicates that the patients implanted with the TC-Dynamic prosthesis have an excellent result. The TC-Dynamic prosthesis with a scientific and proper design is more suitable for the Chinese. However, the long-term outcome of the patients implanted with the TC-Dynamic prosthesis should be observed in a larger number of TKA operations. The basic surgical principles, including excision of both the cruciate ligaments and correction of thebone deformity with the proper balancing of the soft tissues in flexion and extension, are still crucial to successful TKA and to the longterm high survivalrate of the knee prosthesis.

  • FABRICATION OF CUSTOM-MADE ARTIFICIAL SEMIKNEE JOINT BASED ON RAPID PROTOTYPING TECHNIQUE:COMPUTER-ASSISTED DESIGN AND MANUFACTURING

    Objective To design a new custom-made artificial semi-knee joint based on rapid prototyping(RP) technique and to explore a method to solve necroses of allocartilage in hemi-joint allotransplantation. Methods Based on the extracted 3D contour image of the articular cartilage of femoral condyle, the custom-made artificial semi-knee joint was designed with Surfacer 9.0 image processingsoftware. The artificial semi-knee joint design used the femoral condylar 3D contour of the patient as the outer face and the subchondral bone 3D contour of allograft bone as inner face. One dado for medullary nailand two for special designing cages which were used to fix the cartilage into the allograft were added on the inner face. After being converted into RP data format, the computerassisted design was imported into the LPS600 rapid prototyping machine, and the prototype was achieved. Furthermore, the prototype could be modified by hand according to the design. Then the RP model was used as a positive mould to build up a silica gel negativemould, and the negative mould was sent to the factory to manufacture Ti-6Al-4V alloy articular cartilage through ordinary mould-melted founding process. Finally, the whole metal cartilage was completed after melting two special cages on it andpolishing it. Results A new custom-made artificial semi-knee joint was made ad used to treat a 14-year old patient. The custom-made artificial semi-knee joint and the subchondral bone were a perfect match. The operative result was satisfactory. The patient could walk 5 weeks after operation. The bone healing of the auto-bone and allo-bone was achieved 6 months later. A follow-up period lasting 1 yearshowed that the knee joint played a good function. Conclusion The artificial semi-knee joint is a good match for the allograft boneand a good idea to solve necroses of allocartilage in hemijoint allotransplantation. 

  • APPLICATION OF SARTORIUS MUSCLE IN THE QUADRICEPSPLASTY

    OBJECTIVE: Extension stiffness of knee joint is always treated by the quadricepsplasty, but the main deficiency of this method is that patient feels weakness of lower limb and easily kneels down. The aim of this article is to explore the method to resolve the complications after quadricepsplasty. METHODS: Since 1978 to 1997, on the basis of traditional procedures of quadricepsplasty, sartorius muscle was used to reinforce the extension of knee joint. The lower 2/3 of sartorius muscle was fully dissociated only with its insertion intact. A tendon-periosteal-bone flap, about 2 cm in width, was managed on the anterior surface of patella, with its pedicle on the medial edge of patella. The tendon-periosteal-bone flap was used to fix the dissociated sartorious into patella to reinforce the extension of knee joint. The very lower part of sartorius was mainly aponeurosis, with the help of an aponeurosis bundle of iliotibial tract, it was fixed into the insertion of patellar ligament, through a bony tunnel chiseled adjacent to the insertion of patellar ligament. By now the movement of knee joint extension was strengthened by the transferred sartorius muscle. Postoperatively, every patient was required to extend and flex knee joint actively and/or passively. Altogether 12 patients were treated, 9 of them were followed up with an average of 14 months. RESULTS: The average movement was increased from 15 degrees to 102 degrees, and the average myodynamia was improved from grade II to grade IV. CONCLUSION: Traditional quadricepsplasty co-operated with transfer of sartorius muscle can strengthen the myodynamia of knee joint extension. It is simple method and can really achieve good function.

  • FUNCTIONAL RECONSTRUCTION OF HIP AND KNEE AFTER REMOVAL OF TOTAL FEMUR IN TREATING MALIGNANT TUMORS OF MIDDLE OR UPPER THIRD FEMUR

    Abstract From August 1983 to June 1995, 15 cases of middle and upper 1/3 femoral malignant tumors were treated by removal of compete femur and functional reconstruction of both hip and knee. The key procedure was: arrested the artery and vein by rubber band with a nail put in the iliacbone, or exposed the atery and vein directly in the inguinal region. After the disarticulation of the hip and knee, the whole femur with the tumor was removed.Rotated the lower leg 180° and put an artificial femoral head on the upper endof tibia, kept the nerve, artery and vein intact or anastomosed the vessels. Three weeks after surgery, an artificial leg was put on and began to do functionalexercises. From 1~12 years follow-up, no mortality and no local recurrence or metastasis were noted. The function of the reconstructed hip and knee was satisfactory. This procedure could both provide a radical excision of the tumor and obtain a relatively good functioning hip and knee joints.

  • Analysis of 38 Cases of Knee Injury in China Wenchuan Earthquake Wounded

    摘要:目的:探讨地震伤员中膝关节损伤的机制、类型及处理方法。 方法:对我院收入住院的2728例 5·12汶川特大地震伤员伤情特点进行分析,筛选出有膝关节损伤的病例进行分析。 结果: 膝关节损伤前三位分别是髌骨骨折(42.15%)、胫骨平台骨折(21.05%)、股骨髁骨折(13.16%),这与人们在地震时奔跑摔伤以及被房屋倒塌砸伤有很大关系。结论:地震后膝关节损伤其发病率、受伤机制、损伤类型、合并伤情况及治疗处理均有其特点,总结这些特点和规律将对地震伤膝关节损伤患者的处理产生积极作用。Abstract: Objective: To investigate the knee joint injury mechanisms, types and treatment of the earthquake wounded. Methods: Analysis the characteristics of the 2728 cases of West China Hospital from 5.12 Wenchuan earthquake wound, screen out the cases of knee joint injury. Results: Patellar fracture (42.15%),tibial plateau fracture(21.05%), femoral condyle fracture (13.16%), were the top three of knee joint injuries, which have a great relationship with falling down and (or) injured by collapsed houses when people were running in the earthquake. Conclusion: After the earthquake the knee joint damages its disease incidence rate, is injured the mechanism, the damage type, the merge wound situation and treatment processing has its characteristic, will summarize these characteristics and the rule damages patient’s processing to the earthquake wound knee joint to have the positive role.

  • Effect of Arthroscopy to Diagnose and Treat Knee Joint Pigmented Villonodular Synovitis

    摘要:目的:探讨关节镜微创手术对膝关节色素沉着绒毛结节性滑膜炎的诊断和治疗价值。方法:本组12例,男7例,女5例,年龄18~46岁,平均33岁;病史2~60个月,平均16个月;其中左膝8例,右膝4例;初次就诊11例,外院开放手术后复发1例。所有病例术前均行MRI检查,并行关节镜检,滑膜切除,记录该病在关节镜下的表现形式(局灶型或弥漫型),样本全部送病理检查。术后加压包扎、局部冰敷并按计划功能锻炼,术后3~4周行患膝放射治疗。结果:本组12例,其中局灶性病例8例,弥漫性4例,术后病理检查确诊;所有病例获得了3~21个月,平均13个月随访,未见复发;术前Lysholm评分(62.3±2.4)分;国际膝关节评分委员会(IKDC)膝关节功能主观评分(56.4±31)分;术后3月复查Lysholm评分(82.5±3.2)分;IKDC主观评分(85.3±2.5)分。除1例开放手术后复发病例术后3月膝关节屈曲受限(80°)外,其余患者功能良好。结论:关节镜手术创伤小,显露充分,病灶切除彻底,术后功能恢复理想,辅以放射治疗可有效降低复发率,对膝关节色素沉着绒毛结节性滑膜炎具有较高的诊治价值。Abstract: Objective: To evaluate the role of arthroscopy in the diagnosis and treatment in knee joint pigmented villonodular synovitis. Methods: 12 cases of knee joint pigmented villonodular synovitis with the age of 18 to 46 years old were treated with arthroscopical synovectomy with a combined application of postoperative exercise and radiotherapy. The history of disease was 2 to 60 months, with the mean of 16 months. The clinical data were reviewed when followedup and evaluated by Lysholm score and and IKDC score. Results: 12 patients diagnosed by pathologic examination,including 8 localized and 4 diffused, were followed up for 3 to 21 months(13 months on average)with no relapses at the time of followup. Lysholm score was (62.3±2.4)points preoperatively, but (82.5±3.2) points 3 months later.The International Knee Documentation Committee (IKDC) score was (56.4±3.1) and (85.3±2.5) respectively before surgery and 3 months later. All patient remained good functions of knee joints except one who relapsed after open operation. Conclusion:In case of pigmented villonodular synovitis of the knee joint, arthroscopical synovectomy combined with postoperative radiotherapy and physical exercise is an effective treatment with less invasion and better function than open operation.

  • Stress Analysis of Artificial Bionic Knee Joint Based on UG6.0 NX NASTRAN

    This article introduces the basic principles of finite element analysis in biomechanics, focusing on the basic principles of a variety of finite element analysis software, and their respective characteristics. In addition, it also describes the basic stress analysis of UGNX6.0 NASTRAN analysis for artificial knee process, i.e. the choice of the type, material definition, the set of constants, finite element mesh division and the finite element results of the analysis. Finite element analysis and evaluation of the design of personalized artificial knee were carried out, so that the rationality of the geometric design of the structure of the experimental design of artificial knee has been verified.

  • CORRELATION ANALYSIS OF BONE MARROW EDEMA DEGREE AND SERUM INFLAMMATORY FACTORS CHANGE WITH KNEE JOINT PAIN SYMPTOMS IN PATIENTS WITH BONE CONTUSION AROUND THE KNEE JOINT

    ObjectiveTo explore the correlation between the degree of bone marrow edema (BME) and the content change of tumor necrosis factor α (TNF-α) and matrix metalloproteinase 3 (MMP-3) and the knee pain symptoms in patients with bone contusion around the knee joint. MethodsThirty patients (30 knees) of bone contusion around the knee joint were chosen as the trial group between October 2009 and April 2012. According to visual analogue scale (VAS), 30 patients were divided into mild group (10 cases), moderate group (10 cases), and severe group (10 cases); according to MRI morphological changes, the patients were divided into type I group (12 cases), type Ⅱ group (11 cases), and type Ⅲ group (7 cases). Ten patients (10 knees) with soft tissue injury of the knee were chosen as control group. No significant difference was found (P>0.05) in gender, age, causes, side, and admission time after injury between 2 groups. The serum contents of MMP-3 and TNF-α were detected and statistically analysed between different degrees of pain groups and between different degrees of BME groups. Correlation was analysed between BME and inflammatory factor changes and VAS score. ResultsThe MMP-3 and TNF-α contents in trial group[(29.580±6.870) μg/L and (23.750±7.096) ng/L] were significantly higher than those in control group[(8.219±1.355) μg/L and (6.485±1.168) ng/L](t=9.686, P=0.000; t=7.596, P=0.000). The MMP-3 and TNF-α contents in patients with different degrees of pain and BME were significantly higher than those in patients of control group (P<0.05), and significant difference was found between patients with different degrees of pain (P<0.05), but no significant difference between patients with different degrees of BME (P>0.05). Multiple linear regression analysis showed that TNF-α content was significantly correlated with VAS score (P=0.000). ConclusionKnee pain symptoms are not related to the degree of BME in patients with bone contusion around the knee joint. Inflammatory factor TNF-α content is the main influence factor of knee joint pain symptoms.
