  • Effects of Community-hospital Continuous Nursing on the Satisfaction of Day Surgery Patients in Terms of Their Nursing Needs

    Objective To explore the effect of community-hospital continuous nursing on the satisfaction of day surgery patients in terms of their nursing needs. Methods Sixty patients hospitalized in the day surgery ward in our hospital from April 2014 to March 2015 were enrolled in this study. According to the order of admission, they were divided into control group and observation group with 30 patients in each. Patients in the control group were given health guidance, while those in the observation group were given continuous nursing. Questionnaire survey was conducted in both the two groups. The nursing needs of patients after surgery were summarized, and the satisfaction of patients’ nursing needs through different nursing methods were analyzed. Quality of life questionnaire-C30 was used to evaluate the quality of life of the two groups of patients one month and three months after surgery, and the incidence of postoperative complications of the two groups was statistically analyzed. The changes of daily life ability of patients one month and three months after surgery were evaluated by the modified PAP index. Results Day surgery patients had a high demand for postoperative incision care, dressing change, postoperative medication, dietary guidance, postoperative guidance, health education and other aspects of health care needs. There was little demand for oral pipeline care and physical therapy. The satisfaction rate of patients in the observation group toward n ursing service (96.7%) was significantly higher than that in the control group (66.7%) (P < 0.05). The incidence of postoperative wound bleeding, poor healing, infection, urinary retention and other complications in the observation group was lower than those in the control group, but only the difference in the incidence of infection was statistically significant (P < 0.05). One month and three months after surgery, the quality of life in the observation group was significantly better than that in the control group (P < 0.05). The complete self-care ratio in the observation group was higher than that in the control group, and the proportion of slight defect in the observation group was lower than that of the control group (P < 0.05). Conclusion Continuous community-hospital nursing can improve the satisfaction of patients toward nursing service, promote patients’ quality of life, and facilitate patients’ recovery after surgery.

    The social needs, disciplinary development and humanistic reflection has promoted the emergence and development of evidence-based medicine. Moreover, evidence-based medicine-problems oriented research, evidence based decision, transferring results to practice and outcome evaluation-continues to meet society demands, promote the development of discipline and show humanist concern. The application of evidence-based medicine has gradually extended from the field of clinical medicine to the public health, society, management, economy, policy research and education in the process of solving various problems. The high quality evidence has also been important to decision-making in these fields. Our study explored the emergence and development of evidence-based medicine from the perspective of social needs, disciplinary development and humanistic reflection for the first time.

  • A longitudinal study on supportive care needs and quality of life of patients with thyroid cancer

    ObjectiveTo investigate the supportive care needs (SCNs) and quality of life (QOL) of patients with thyroid cancer at different time points, and explore the trends and relation between the two. MethodsFrom April 2020 to July 2020, the convenience sampling method was used to select the patients with thyroid cancer that met the criteria, and the general data were collected before discharge. The 34-item Supportive Care Needs Survey and the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30 were used to investigate the SCNs and QOL of patients at preoperative 1 d and postoperative 2 d, 1 month and 6 months. ResultsA total of 102 patients met the criteria were included. Total score of SCNs (F=13.407, P<0.001) and the points of psychological (F=38.525, P<0.001), health information (F=7.935, P=0.006), physical and daily living (F=56.413, P<0.001), patient care and support (F=40.530, P<0.001) needs were statistically different at each time point. The highest need was the psychological need at preoperative 1 d, which was the health information need at each time point after operation, and total score of SCNs was the highest at postoperative 2 d. The points of physical function (F=18.490, P<0.001), cognitive function (F=86.943, P<0.001), emotional function (F=9.121, P=0.003), social function (F=7.117, P=0.009), and overall health status (F=3.156, P=0.039) were statistically different at each time point, which of the physical function, role function, emotional function, social function, and overall health status decreased to the lowest on day 2 after operation. The total score of SCNs at each time point was negatively correlated with the functional factors of the QOL and the overall health status score (P<0.05). ConclusionsThe SCNs and QOL of patients with thyroid cancer show different trends at different time points, and there is a correlation between the two. Medical staff needs to develop personalized interventions according to the different stages of the patients’ disease, and actively provide targeted support and care so as to improve their QOL.

  • Current status of caregivers' knowledge and demand for wearable seizure detection devices in children with epilepsy

    ObjectiveThis study aims to assess the current state of caregivers' awareness and demand for wearable seizure detection devices in children with epilepsy. MethodsA convenience sampling approach was employed to select 207 caregivers of children with epilepsy, who met the predefined inclusion and exclusion criteria at Shenzhen Children's Hospital between April and June 2024. Data were gathered through general demographic surveys and a specialized questionnaire assessing caregivers' recognition of and demand for wearable seizure detection devices. ResultsThe findings revealed that the majority of caregivers were largely unaware of wearable seizure detection devices, yet exhibited substantial concern over the potential for undetected seizures. Notably, a significant proportion of caregivers expressed a keen interest in adopting such devices and demonstrated a willingness to use them on a long-term basis. A moderate positive correlation was observed between caregivers' apprehension about undetected seizures and their level of interest in using wearable monitoring devices (r=0.455, P<0.001). Furthermore, most caregivers indicated a willingness to accept devices with false alarm and false negative rates of ≤25%, preferring to receive seizure alerts within one minute of onset. The majority of participants also expressed that they would only consider using the device if its cost were covered by insurance. ConclusionCaregivers of children with epilepsy exhibit significant concern over undetected seizures, coupled with limited awareness of wearable seizure detection devices. However, they express strong interest in adopting such technologies. These findings underscore the imperative to continue advancing the functionality and accessibility of these devices, enhance public awareness, and further educate caregivers to foster greater confidence and trust in wearable epilepsy monitoring solutions.

  • Characteristics and rehabilitation needs of patients injured in “8·8” Jiuzhaigou earthquake

    Objective To analyze the rehabilitation needs and characteristics of patients injured in Jiuzhaigou earthquake, and provide early rehabilitation guidance for the wounded. Methods A total of 42 patients injured in " 8·8” Jiuzhaigou earthquake who were admitted to West China Hospital from August 9th to 16th 2017 for treatment were recruited in this study. Early survey and evaluation were conducted among them by using a self-made questionnaire. Results The top 2 common causes of injury were falling (23 cases, 54.8%) and flying stone (16 cases, 38.1%). The top 2 common injury sites were lower extremity (18 cases, 42.9%) and head (10 cases, 23.8%). Twenty-two cases (52.4%) left the hospital after treatment, 6 (14.3%) went to the Department of Orthopedics, and 6 (14.3%) went to the Department of Neurosurgery. The functional limitations included sensory function in 42 cases (100.0%), mental function in 28 (66.7%), motor function restriction in 24 (57.1%), balance physical limitation in 22 (52.4%), respiratory function in 5 (11.9%), and cardiac function in 1 (2.4%). A total of 37 cases (88.1%) had restricted activities of daily living capacities, mainly including 24 (57.1%) in stair climbing, 24 (57.1%) in walking and 22 (52.4%) in bathing. All the 42 cases needed functional evaluation: 22 (52.4%) needed rehabilitation treatment, and 19 (45.3%) needed rehabilitation aids. Conclusions The injury and function limitation of Jiuzhaigou earthquake victims has its own characteristics. According to the limitation of function, the direction of rehabilitation plan after the earthquake focuses on structural function, psychological function, daily activities and function rehabilitation, so as to bring patients back to the family and society as early as possible.

  • Investigation and analysis of preoperative cognition and health education service needs of ophthalmic fundus day surgery patients

    ObjectiveTo investigate the preoperative cognition of the patients undergoing daytime ophthalmic fundus surgery and understand their needs of health education, so as to provide an evidence for efficient and accurate preoperative health education services within the limited time of the ophthalmic day fundus surgery.MethodsThe convenient sampling method was used to select the patients who met the inclusion criteria in the ambulatory operating room of Beijing Tongren Hospital, Capital Medical University from December 2017 to May 2018. The study included three parts: the general information of the patients, the preoperative cognition of the patients, and the needs for health education service of the patients. Questionnaires were designed according to the research purpose and method, which were distributed and recovered by professionals.ResultsA total of 112 patients were included. Among them, the cognitive scores of operation process (2.57±0.56), preoperative diet (2.58±0.59), preoperative medication (2.60±0.64), and psychological status (2.58±0.65) were relatively low. More health education services were needed in three aspects: the cognition of operation details [operation duration (85.71%), surgeons (79.46%), operation start time (76.79%)], intraoperative cooperation (90.18%), and intervention for preoperative anxiety (78.57%).ConclusionNurses should formulate the contents of preoperative health education according to the preoperative cognition and nursing needs of patients, so as to provide efficient and accurate health education services for patients.

  • Exploration and evaluation of a new model of scientific research management based on clinical needs

    Objective To analyze the scientific research output of West China Hospital of Sichuan University from 2015 to 2020, so as to evaluate the effectiveness of new scientific research management model. Methods From January 2018, a series of management measures were introduced, including improving research incentive policies, integrating and supporting resources, constructing a scientific quantitative research performance evaluation system, and establishing an information-based research service model. The changes of scientific research output before (2015-2017) and after (2018-2020) the implementation of the management measures were analyzed to evaluate the quality and effectiveness of the management model. Results A total of 10141 high-quality research papers were seized and 2311 research projects were approved. After the implementation of the measures, the absolute number of papers published and projects approved increased significantly. In terms of high-quality papers published after the implementation of the measures, the increase rate reached 46.80%, among which the number of interdisciplinary papers increased by 116.05%, the number of reviews increased by 96.13%, the numbers of papers published in Area 2 and Area 1 increased by 114.67% and 75.00%, respectively, and the number of co-author papers increased by over seven times. In terms of scientific research projects approved after the implementation of the measures, the number of projects approved by the National Natural Science Foundation of China (NSFC) increased by 21.03%, and the number of provincial and ministerial projects increased by 110.17%, with the people in charge becoming younger. The number of principal investigators of NSFC increased by 1.67 times. The number of projects managed by intermediate and undetermined professional titles increased by 64.71% and 68.63%, respectively. The number of projects approved by youth and general fund increased by 30.26% and 14.16% respectively. The number of people in charge of provincial and ministerial projects increased by 1.45 times. The number of projects managed by deputy senior and intermediate titles increased by 1.64 times and 1.54 times, respectively. Key research and development projects, key projects, international cooperation projects, and basic application projects saw a significant increase. Conclusions After the implementation of management measures, the number and overall level of research papers have been improved, and the cross-cooperation between clinical and basic research has been gradually started. The number of projects approved increased significantly, and the project leaders tended to be younger. The high-quality cultivation and continuous growth of scientific research talents achieved initial results.

  • A Survey on Care Needs of Outpatients in Stomatological Hospital based on Kano Model

    ObjectiveTo assess the care needs of outpatients in stomatological hospital and provide evidence for the improvement of patients' satisfaction. MethodsAccording to the characteristics of outpatients in stomatological hospital, a convenience sample of 150 cases was assessed with care needs survey based on Kano Model between May 2011 to February 2012. ResultsAmong all the care needs, 6 were essential, 11 were expected, and 3 had an attractive quality. Essential and attractive care needs accounted for 70% of all items. In the quadrant chart analysis, there were 8 items in competitive advantage district and 12 in the urgent improvement district respectively. ConclusionKano model is a quality attributive recognition tool. Through the analysis of the questionnaire, we believe the hospital should improve such aspects as information disclosure, public facilities, appointment, optimization process, and shortening patients' waiting time, in order to meet patients' needs by providing them with high quality services.
