  • Effectiveness of Cognitive Behavioral Therapy Treating Insomnia in Patients with Breast Cancer: A Systematic Review

    Objectives To evaluate the effectiveness of cognitive behavioral therapy (CBT) treating insomnia in patients with breast cancer. Methods Such databases as CENTRAL (April 2011), The Cochrane Library (2005 to November 2011), MEDLINE (1948 to April 2011), EMbase (Junuary 1966 to April 2011), CINAHL (Cumulative Index to Nursing and Allied Health Literature, 1982 to April 2011), CBM (1978 to 2011) and CNKI (1979 to 2011) were searched to collect the randomized controlled trials (RCTs) about CBT treating insomnia in patients with breast cancer. Literature screening, data extraction and methodological quality assessment of the included studies were conducted according to the inclusion and exclusion criteria. Then RevMan 5.0 software was used for meta-analysis. Results A total of 5 RCTs were included. The results of meta-analysis showed that, compared with the control group, the CBT group was higher in the score of subjective sleep efficiency; however, it was lower in the subjective sleep onset latency. There were no significant differences between the two groups in the sleep efficiency, total sleep time, sleep onset latency and wake time after sleep onset measured by hypnocinematograph. Conclusion Cognitive behavioral therapy may improve the subjective sleep efficiency and sleep quality, and shorten the sleep onset latency in patients with breast cancer. However, its effect of improving objective sleep quality is still unverifiable at present. It suggests the following measures should be taken for providing better evidences: increasing sample size, adopting blind methods, confirming allocation concealment, and using unified hypnocinematograph.

  • COMPARISON OF FEMORAL CONDYLAR TWIST ANGLE IN THREE DIMENSIONAL RECONSTRUCTION DIGITAL MODELS OF KNEE JOINT BASED ON TWO DIMENSIONAL IMAGES OF MRI AND CT

    ObjectiveTo study the difference of femoral condylar twist angle (CTA) measurement in three dimensional (3-D) reconstruction digital models of human knee joint based on the two dimensional (2-D) images of MRI and CT so as to provide a reference for selecting the best method of CTA measurement in preoperative design for the femoral prosthesis rotational position. MethodsThe CTA of 10 human cadaveric knee joint was measured in 3-D digital models based on MRI (group A), in 3-D digital models based on CT (group B), in the cadaveric knee joint with cartilage (group C), and in the cadaveric knee joint without cartilage (group D), respectively. The statistical analysis of the differences was made among the measurements of the CTA. ResultsThe CTA values measured in 3-D digital models were (6.43±0.53)° in group A and (3.31±1.07)° in group B, showing significant difference (t=10.235, P=0.000). The CTA values measured in the cadaveric knee joint were (5.21±1.28)° in group C and (3.33±1.12)° in group D, showing significant difference (t=5.770, P=0.000). There was significant difference in the CTA values between group B and group C (t=5.779, P=0.000), but no significant difference was found between group A and group C (t=3.219, P=0.110). ConclusionThe CTA values measured in the 3-D digital models based on MRI are closer to the actual values measured in the knee joint with cartilage, and benefit for preoperative plan.

  • PRELIMINARY STUDY ON EFFECT OF FEMORAL TUNNEL ANGLE ON FEMORAL TUNNEL AFTER ANTERIOR CRUCIATE LIGAMENT RECONSTRUCTION IN RABBITS

    ObjectiveTo investigate the effect of the femoral tunnel angle on the femoral tunnel after anterior cruciate ligament (ACL) reconstruction in rabbits. MethodsFifty-four healthy 4-5 months old rabbits (weighing, 1.8-2.3 kg, male or female) were randomly divided into 3 groups (n=18). The ACL reconstruction models of the right knee were established in 3 experimental groups using its Achilles tendons, and the left knee served as the control group. On the coronal position, the angle between the femoral tunnel and the femoral shaft axis was 30°, 45°, and 60°. The level of tumor necrosis factor α (TNF-α) in the synovial fluid at 1, 2, and 4 weeks, the maximum load of the ligament and the rate of bone tunnel enlargement at 4, 8, and 12 weeks were detected. ResultsThe level of TNF-α significantly increased, and the maximum load of the ligament significantly decreased in the 3 experimental groups when compared with ones in the control group (P<0.05), but no significant difference was found among 3 experimental groups (P>0.05). The bone tunnel enlargement was observed in 3 experimental groups at each time point and reached the peak at 4 weeks, but no significant difference was shown among 3 groups (P>0.05). ConclusionThe 30-60° angle between the femoral tunnel and the femoral shaft axis in the coronal position has no significant effect on the femoral tunnel enlargement after ACL reconstruction in rabbits.

  • EFFECTS OF LEUCOCYTE- AND PLATELET-RICH PLASMA ON OSTEOGENIC DIFFERENTIATION OF BONE MARROW MESENCHYMAL STEM CELLS IN TREATING AVASCULAR NECROSIS OF FEMORAL HEAD IN RABBITS

    ObjectiveTo evaluate the effect of leucocyte- and platelet-rich plasma (L-PRP) on the osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs) in treating avascular necrosis of the femoral head (ANFH) in rabbits. MethodsTwenty-four New Zealand white rabbits (4-6 months old, both genders, weighing 2.0-3.0 kg) were used for the establishment of bilateral ANFH models and divided into 4 groups (n=6). BMSCs were isolated from the bone marrow of iliac crest, cultured and identified. L-PRP was prepared by Landesberg method. Core decompression only (group A), core decompression and L-PRP implantation (group B), core decompression and BMSCs implantation (group C), and core decompression and implantation of BMSCs and L-PRP were performed in 4 groups. To evaluate bone formation and remodeling of the defects, X-ray photography was taken at 2, 4, and 8 weeks postoperatively. The modified Lane-Sandhu scoring system was used to evaluate the bone formation. Two rabbits were sacrificed at 2, 4, 8 weeks after operation to harvest the specimens for histological observation, new blood vessel count and new bone area ratio. ResultsThe observations of radiology and histology displayed different degrees of bone regeneration at bone defect sites in each group. At 2, 4, and 8 weeks postoperatively, the results of Lane-Sandhu X-ray photography scoring, new blood vessel count, and new bone area ratio showed that groups C and D were significantly better than groups A and B, group D was significantly better than group C. and group B was significantly better than group A (P<0.05). ConclusionThese findings demonstrate that L-PRP can promote osteogenic differentiation of BMSCs in treating ANFH in rabbits, and core decompression associated with BMSCs and L-PRP is an effective and feasible method to treat ANFH.

  • Medical Education Model with Core Competency as Guide, Evidence-Based Medicine as Carrier and Life Learning as Purpose (4): Effects of Evidence-Based Medicine Course on Improvement of Information Consciousness and Morality for Medical Students

    Objective To explore the effects of evidence-based medicine (EBM) course on improvement of information consciousness and information morality for medical students.Methods A total of 288 medical undergraduates and postgraduates, who took EBM as an elective course in Sichuan University, were surveyed with ‘information consciousness and information morality questionnaire’ before and after that course. Results After the EBM course, the number of students surfing the Internet increased by 5%, of which postgraduates increased by 24% with significant differences (Plt;0.05), while their purpose for specialized knowledge increased by 7%; the number of students making plans in advance increased by 7%, and the number of students learning intellectual property and copyright law increased by 7%; the number of postgraduates knowing how to cite articles increased by 12% with significant differences (Plt;0.05); and the number of students not knowing how to get legitimate information decreased by 12% with significant differences (Plt;0.05). Conclusion Evidence-based medicine course is positive for the information consciousness and information morality of medical students.

  • An Epidemiology Study on Child Behavior Problems among High-Grade Primary School Students in Chongqing Urban Area

    Objective To explore the prevalence and related factors of behavior problems among the high-grade primary school students (in Grade 4 to Grade 6) in Chongqing urban area. Methods By means of cross-sectional survey, a total of 3 137 high-grade primary school students in Chongqing urban area were investigated with Child Behavior Checklist (CBCL, version for parents) and self-designed basic information questionnaire. Results The relevance ratio of behavior problems was 27.26%. The univariate analysis revealed that type of school, grade, residential and family environment, and parental attitude toward children had effects on children’s behavior problems with significance difference (Plt;0.05). The stepwise regression revealed that grade, educational level of father, healthy conditions of parents, family structure, academic performance and parental attitude toward children were the independent and main influencing factors (Plt;0.05). Conclusion The relevance ratio of behavior problems among high-grade primary school students in Chongqing urban area is much higher, which is related to the following factors: family situation, parental attitude toward children, social and school environment. It indicates that the status of children’s mental heath is not optimistic in Chongqing urban area. Comprehensive measures combining family, school with psychologist should be taken to reduce the prevalence of behavioral problems.

  • Arthroplasty Compared with Internal Fixation for Displaced Femoral Neck Fractures in the Elderly: A Meta-Analysis

    Objective To compare the effects of arthroplasty with that of intenal fixation for displaced femoral neck fractures in the elderly. Methods We searched for all randomized controlled trials and quasi-randomized controlled trials of hip arthroplasty versus internal fixation for displaced femoral neck fractures in the elderly by electronically searching MEDLINE( 1966 to September, 2006),EMbase(1966 to September, 2006), Cochrane Library( Issue 4 2006), CBM( up to September, 2006) and CNKI (September, 2006) and handsearching grey literatures. The quality of the trials was assessed and meta-analyses were conducted using The Cochrane Collaboration’s RevMan 4.2.8 software. The quality of the trials was assessed. And meta-analysis was conducted by using RevMan4.2.8 software. Results A total of 228 papers were retrieved, but only 15 published randomized controlled trials involving a total of 2 254 patients were suitable for inclusion in the review. Based on the meta-analyses, hip arthroplasty showed obvious advantages over internal fixation in terms of the incidences of major complications after 2 years (RR 0.15 ; 95%CI 0.09 to 0.23) and after 5 years (RR 0.18 ; 95%CI 0.11 to 0.30) as well as re-operation rate (RR0.12 ; 95%CI 0.08 to 0.18) after 2 years, and after 5 years (RR0.11 ; 95%CI 0.06 to 0.22), there was an advantage to performing hip asthroplasty. After one year, the mortality was the same in both groups with RR 1.05 and 95%CI 0.89 to 1.23. Conclusion There is an evidence base to support arthroplasty as a treatment for displaced femoral neck fractures in the elderly. Arthroplasty can not only decrease the rate of re-operation, but can also reduce the incidence of complications, with similar one-year mortality when compared to internal fixation.

  • Wound infectiousness pseudo femoral artery lump treatment experience

    摘要:目的:探讨创伤性感染性假性股动脉瘤的诊断和外科治疗的临床经验。方法:回顾性分析21例创伤性感染性假性股动脉瘤的临床资料,均行瘤体摘除及彻底的清创后,分别采用了血管结扎术和血管修复重建术两种不同的手术治疗方法。结果:血管修复重建术组中3例术后出现血管破裂大出血,要再次手术,15例行股动脉结扎术,全部保肢成功。结论:瘤体切除加血管移植术是一种理想的方法,但在无条件行血管移植时,股动脉结扎术可做为一种有效的方案,对伴有皮肤缺损者行对侧胸脐皮瓣转移术。Abstract: Objective: 〖WT5”BZ〗To explore the traumatic infected femoral pseudoaneurysm diagnosis and surgical treatment of clinical experience. Methods: Retrospective analysis of 21 cases of traumatic infected femoral pseudoaneurysm of the clinical data were performed and the tumor removed after thorough debridement, respectively vascular ligation and blood vessel repair and reconstruction surgery of two different surgical treatment. Results: The blood vessel repair and reconstruction surgery group, three cases of postoperative bleeding blood vessel ruptures occurred, we must resurgery, 15 routine femoral artery ligation, all of the success of limb salvage. Conclusion: The tumor resection plus vascular graft is an ideal way, but in an unconditional line of vascular grafts, the femoral artery ligation can be used as an effective program for skin defects associated with the contralateral breast underwent umbilical flap transfer of patients.

  • Differential Diagnosis of Inguinal Hernia and Femoral Hernia by Multi-Detector Row CT

    Objective To assess the significance of multi-detector row CT in differential diagnosis of the inguinal hernia and femoral hernia. Methods CT images which were reconstructed by multi-planer reconstruction (MPR) of 260 patients with inguinal hernia and femoral hernia who treated in our hospital form Oct. 1, 2012 to Oct. 31, 2013 were analyzed retrospectively, for exploring the relationship between sac and anatomic structure in the groin area. Results There were 146 patients with indirect hernia (75 in right, 60 in left, and 11 in bilateralism), 82 patients with direct hernia(39 in right, 34 in left, and 9 in bilateralism), and 32 patients with femoral hernia (17 in right and 15 in left). The 157sacs of patients with indirect hernia originated lateral to the inferior epigastric artery, entered the inguinal canal and through the deep ring, which mainly located anterior (103/157, 65.6%) or anteromedial (36/157, 22.9%) to the spermatic cord or round ligament. The 91 sacs of patients with direct hernia originated medial to the inferior epigastric artery, and mainly located medial to the spermatic cord (70/91, 76.9%). Sacs of both indirect hernia and direct hernia located anterosuperior to the inguinal ligament. The 32 sacs of patients with femoral hernia located posterior to the inguinal ligament and inside the “radiological femoral triangle” of coronal views. Conclusions The MPR images available from multi-detector row CT permit the accurate diagnosis of groin hernias. By using simple anatomical criteria, direct hernia, indirect hernia, and femoral hernia can be reliably distinguished.

  • Subfascial Endoscopic Communicating Branch Vein Ablation in Treating Communicating Branch Vein Insufficiency (Report of 57 Cases)

    bjective To study the effectiveness of subfascial endoscopic communicating branch vein ablation in treating communicating branch vein insufficiency.MethodsFiftyseven cases of primary lower extremity vein valve insufficiency complicated by communicating vein insufficiency diagnosed by venography and color duplex were treated with subfascial endoscopic communicating branch vein ablation, and their information was analyzed retrospectively.ResultsSwelling was observed in leg of 7 cases after operation, and disappeared within one week spontaneously. Incision infection occurred in 2 cases and skin ambustion happened in 2 cases. Superficial vein varicosis disappeared in all case, skin pigmentatin declined significantly and ulcers healed in 2-3 weeks.Conclusion Subfascial endoscopic communicating branch vein ablation is an excellent choice in treating communicating branch vein insufficiency.
