Primary palmar hyperhidrosis is a kind of benign disease characterized by abnormally increasing sweat. Various treatments for it exist in clinic currently and the one of them is endoscopic thoracic sympathectomy. But due to the different choice of surgery path, surgery method and the way or the level to block the sympathetic chain, the surgical curative effect and the incidence of postoperative complications vary hugely. This review provides an update report on the surgery method and the way or the level to block the sympathetic chain to treat palmar hyperhidrosis by endoscopic thoracic sympathectomy.
ObjectiveTo observe the OCT image characteristics of the posterior cortical anterior vitreous capsular bag (PPVP) in human eyes.MethodsA retrospective clinical study. From May 2012 to February 2017, 107 PPVP patients (173 eyes) diagnosed by OCT in Ophthalmology Department of The Third People's Hospital of Chengdu were included in the study. One hundred and three eyes underwent examination of frequency-domain OCT (SD-OCT) and 70 eyes underwent examination of frequency-scanning light source OCT (SS-OCT), respectively. SD-OCT the German Heidelberg Spectralis OCT instrument. SS-OCT was performed by the vitreous enhancement scan mode of Japan's Topcon DRI OCT instrument. The differences of different types of PPVP OCT imaging and image characteristics were observed.ResultsSD-OCT and SS-OCT showed that the PPVP structure was clear and manifested as macular weakly reflective in the shape of a boat-shaped cavity, the front boundary was vitreous collagen, and the rear boundary was vitreous cortex. There was a weakly reflective Martegiani area on the nasal side of PPVP, and there was a connection channels between them. The two examinations showed a similar pattern, but SS-OCT with the clearer imaging of PPVP.ConclusionsThe OCT image of PPVP in the human eye is characterized by an anterior weakly reflecting boat-shaped cavity in the macula, with anterior border of vitreous collagen and posterior border of vitreous cortex. Martegiani area is located on the nasal side.
Objective To investigate the prevalence and related factors of primary palmar hyperhidrosis in adolescents in Yangzhou. Methods On-site questionnaire survey was performed on students selected by cluster random sampling from the two colleges and two high or middle schools, with each class as a unit. Data were collected through the questionnaire to make the diagnosis and severity grading. Results A total of 3 487 copies of the questionnaire were distributed in the survey and 3 299 were finished, among which 3 083 were effective with an effective rate of 88.41%. Among them, 1 358 respondents were males and 1 725 were females; 933 were middle school students, 809 high school students, and the remaining 1 341 college students. According to the diagnostic criteria, 104 respondents were diagnosed with palmar hyperhidrosis with an overall prevalence of 3.37%. There were 60 (4.41%) males and 44 (2.55%) females. Although the prevalence of palmar hyperhidrosis in males was higher than that of females (χ2=8.130, P<0.05), severe palmar hyperhidrosis was more often to be observed in females than in males, and females were also more likely to have hyperhidrosis in other parts of the body. In addition, the age of the first onset of the disease was mainly 10 to 20 years old and 36.54% of the patients had a family history. Conclusion The prevalence of palmar hyperhidrosis in adolescents in Yangzhou was 3.37%, and there is a significant difference in the gender. The palmar hyperhidros is often accompanied by hyperhidrosis symptoms of other parts of body, and the disease shows an obvious genetic predisposition.
Abstract: Objective To evaluate the treatment efficacy of post-infarction left ventricular pseudo-aneurysm (LVPA) through surgical procedure, and explore the diagnosis and differential diagnosis details of LVPA. Methods Between May 1993 and July 2007, 7 cases were diagnosed through echocardiography aided with left ventriculography or multi-sliced computer tomography (MSCT) or magnetic resonance imaging (MRI); 6 cases with LVPA were surgically treated through different procedure that included direct closure, cut and patching or cut and sandwiching procedure choose according to its location, anatomical morphology, and comorbidity; accompanied diseases were treated by coronary artery bypass grafting(CABG) procedure. Results Six cases were diagnosed before surgery, and 1 case was diagnosed during the surgical procedure. One died from the cardiac tamponade due to rupture of LVPA before the surgical procedure, so the inhospital mortality was 14.3%(1/7). There was no operative death. With the follow-up from 2 months to 13 years of the 6 operational survivors, 1 case died from cardiac rupture and pericardial tamponade 4 years after the repair procedure. Of the 5 surviving LVPA, the left ventricular ejection fraction(LVEF) values were from 43% to 52%, and 3 cases were in New York Heart Association (NYHA) class Ⅰ, and 2 cases were in NYHA class Ⅱ. Conclusion Echocardiography, aided with left ventriculography or MSCT or MRI, is an effective measure for diagnosis of LVPA. Surgical procedure is an effective measure to treat LVPA,but different surgical procedures, accompanied with homeochronous CABG procedure,should be adopted to deal with LVPA according its location, anatomical morphology, and accompanied deformity. The perioperative and mid-long term efficacy were good for the surgical treatment of LVPA, but it is imperative to pay attention to prevention of the recurrence and the late rupture of repaired LVPA.
