Objective To introduce the research update of microencapsulation and its application in orthopedics. Methods Recent articlesconcerned were extensively reviewed. Results Drugs and cells modified by genecould be encapsulated in different materials and be implanted in vivo avoiding a host immune system rejection. It act as a continuous source of desired medicine for enhancement of bone healing, the treatment of bone tumor and bone infection, and the regeneration of bone and cartilage. Conclusion Microencapsulation can be used asa carrier for drugs and cells modified gene to treat related disease in orthopedics.
ObjectiveTo explore the innovative application and effect of lead-type close medical consortium under the epidemic situation of coronavirus disease 2019 (COVID-19).MethodsDuring the epidemic of COVID-19, Jintang First People’s Hospital implemented a series of innovative countermeasures under the guidance of West China Hospital of Sichuan University, the leading hospital of a lead-type close medical consortium. To verify the implementation effect of the countermeasures, the patient satisfaction questionnaire and medical personnel satisfaction questionnaire were administered in confirmed or suspected COVID-19 patients and anti-epidemic healthcare workers. The relevant health economic indicators were extracted through the hospital information system for descriptive analysis.ResultsA total of 16 patients were included, including 10 confirmed patients and 6 suspected patients. The median score of patient satisfaction was 66. All patients were cured and discharged. A total of 56 healthcare workers were included, including 18 doctors and 38 nurses, with a median satisfaction score of 81. The average length of hospital stay of patients was 11.00 d, the average hospitalization cost was 5 117.35 yuan; the average drug cost was 1 099.95 yuan, accounted for 21.49%; the average material cost was 38.63 yuan, accounted for 0.75%.ConclusionsThe innovative application of the lead-type close medical consortium in the treatment of patients with COVID-19 plays an important role in the prevention and control of epidemic, and has achieved remarkable results in patients treatment, patients satisfaction, and hospital management. It is worthy of being widely popularized.
Objective To investigate the growth of the tissue engineered mucosa after its heterotransplantation. Methods The epithelial cells and fibroblasts were isolated from a postoperative tissue of the 3month patient with labialcleft. The epithelial cells and fibroblasts were separately seeded on the polylactic/glycolic acid copolymer membrane, and then they were exposed to the air-liquid interface. Seven volunteer patients, whose traumatic beds were repaired with the tissue engineered oral mucosa. The biopsy tissue from one of the seven patients was observed under light icroscope 18 and 30 days after transplantation, respectively. Results The tissue engineered oral mucosa having 5-6 layers anticytoeratin staining positively cells in the epithelial layer and 3-7 layers anti-Cytoeratin staining negatively cells in the subepithelial layer grew well after the he terotransplantation. No differ ence could be found between the transplanting and normal areas. At 18 days, the epithelial layer and lamina propria grew well and the fibroblasts were found; at 30 days, collagen was obviously observed. The structures in both the transplanting and the normal areas were similar. Conclusion The tissue engineered oral mucosa can grow well after the heterotransplantation.
Since smart phones have been developed, significant advances in the function of mobile phone due to the development of software, hardware and accessories have been reached. Till now, smart phones have been engaged in daily life with an increasing impact. As a new medical model, mobile phone medicine is emerging and has found widespread applications in medicine, especially in diagnosing, monitoring and screening various diseases. In addition, mobile phone medical application shows great potential trend to improve healthcare in resource-limited regions due to its advantageous features of portability and information communication capability. Nowadays, the scientific and technological issues related to mobile phone medicine have attracted worldwide attention. In this review, we summarize state-of-the-art advances of mobile phone medicine with focus on its diagnostics applications in order to expand the fields of their applications and promote healthcare informatization.
Objective To introduce the appropriate application of statistical analysis method in medical science and technology articles. Methods Expatiated the correct application of statistical theories in statistical package, statistical analysis methods and test criterion which compose the basic statistical content in an article.Results If the distribution of numerical variable is normal, mean and standard deviance can be used to describe this variable. In the same way, t test and analysis of variance (ANOVA) can be used to test the difference of mean in each group. If it is not normal, median and range can be used to describe the variable and rank sum test can be used to test the difference of distribution in each group. Categorical variable can be described by rate, proportion and ratio. There are chi-square test, fisher’s exact test and ranksum test to test the difference of rates. Conclusion It is the key of choosing rational statistical methods to distinguish the type of design and variable.
There are a great number of uncertainties in medical practice, causing considerable difficulties in medical activities such as diagnosis and prognostic prediction. Neural-fuzzy system (NFS) combines the advantages of artificial neural networks and fuzzy logic very well, and has become a new type of artificial intelligence model which is capable of acquiring knowledge from data and expressing it in the form of fuzzy rules. Because of its strong capability of classification and processing fuzzy information, NFS is more and more used in medical practice. Adaptive neural-fuzzy inference system (ANFIS) is one of the most popular forms of NFS. This review focuses on the use of ANFIS in medical practice.
Objective To investigate the application value of spiral CT postprocessing technique in the airway stent implantation technology. Methods Twenty-three patients with malignant airway stenosis or malignant tracheoesophageal fistula who needed the treatment of airway stent implantation from May 2012 to April 2016 were collected, including 19 males and 4 females with an average age of (61.6±10.0) years. Bronchoscopy and spiral CT with three-dimensional airway reconstruction were proceeded for the same patients before and after stent implantation, and the relevant data about narrow airway was measured by volume rendering, multiplanar reformation (MPR) and CT virtual endoscopy (CTVE) in a variety of ways, to confirm the location and size of narrow airway and fistulas, the degree and length of airway stenosis, as well as the distal end of the situation, and evaluate the patency of airway, the position and shape of stents, adjacent airway and complications after stenting. The positive forecast outcomes of the two inspections wascompared. Results Airway stents were placed successfully according to the data from the spiral CT airway three-dimensional reconstruction. Thirty stents were implantated in the 23 patients, including 21 column-type stents, 3 L-type stents, and 6 Y-type stents. All stents stayed in situ, with patency and no deformation.The fistulas were closed and the airways were reopened. Symptoms of cough after eating and drinking and dyspnea were relieved. The positive rates of bronchoscopy and CT examination on diagnosis of airway stenosis were both 100% (23/23). Complications: MRP showed tumor growth leading to stenosis again in 3 patients, and CTVE displayed mucous congestion in 2 patients. Conclusions The technique of 64-layer spiral CT postprocessing technique can measure the relate data of airway stricture or fistulas as a kind of convenient, quick, accurate, and noninvasive method in patients with malignant airway stenosis or tracheoesophageal fistula who need the treatment of airway stenting. It is of high reference value both to airway stent implanting and postoperative observation, and is worthy of application.
Objective To investigate the factors that affect platelet-rich plasma (PRP) in promoting bone regeneration and repairing. Methods Recent l iterature was reviewed, concerning the preparations of PRP, physiological mechanism and the latest appl ications in orthopedic field. Results PRP, the concentrated body of autologous platelet, was rich in platelets and was the source of autologous growth factors. Many studies had shown that PRP played an important role in promoting bone regeneration and repairing. However, a few experimental results contradicted this point. The reason might be that the biological properties of PRP were influenced by various factors, such as workmanship, vector, activation schemes, working concentration, individual difference. Conclusion The concentration and qual ity of platelet and other related factorsof PRP affect the rel iabil ity of the results and conclusions. So an efficient and stable production method of PRP should beestabl ished.
Objective To observe clinical effects of burn wounds treatment with bovine amnion and to screen the best method of preparing and storing of bovine amnion. Methods From January 2004 to January 2005,We selected randomly 58 patients with superficial Ⅱ° wound, deepⅡ° wound, autografting area for removal of eschars and tangential excision, fetching skin area or residual burn wound . Using auto-control, every burn wound was divided into 3 parts and was treated with 3 dressings: bovine amnion dealt with by 0.1% chlorhexidine(group A), bovine amnion dealt with by 0.4% glutaraldehyde(group B) and vaseline gauze dressing(group C as control). The clinical effects were compared between different groupsand the method of preparing and storing bovine amnion was evaluated. Results The dressing texture of group A was softer than that of group B, and its flexibility was fine. The pretreatment was not necessary for dressing in group A. When the dressing was used on burn wounds in groups A and B, painwas slight, but pain was obvious in group C; healing time in groups A and B was much less than that in group C, showing statistically significant difference(P<0.01). There was no statistically significant difference in healing time between groups A and B (P>0.05). The infection ratio of burn wound in deepⅡ° wound and residual burn wound of groups A and B is much lower than that of group C, showing statistically significant difference (P<0.05); in theother burn wounds there was no significant difference (P>0.05). There was no statistically significant difference between groups A and B (P>0.05). Conclusion Bovine amnion could make benefit on burn wounds healing, reduce infection ratio of burn wounds, could be used on different kinds of burn wounds. The clinical effect between bovine amnion dealt with by glutaraldehyde and by chlorhexidine is similar. Whereas the latter is more easy to be popularized.
Grading the evidence of systematic reviews on animal studies will contribute to the improvement in the feasibility of transforming the results of animal studies into clinical trials or clinical practice. High quality evidence from animal studies is more likely to be successfully applied into clinical practice (i.e. more confident). Therefore, the present study will introduce the principles, methods and challenges of the application of GRADE in systematic reviews on animal studies.
