Abstract: Objective To summarize the clinical experiences of performing extracorporeal membrane oxygenation (ECMO) on patients with heart and respiratory failure, and compare the clinical outcomes of Medtronic system and Quadrox PLS system. Methods We retrospectively analyzed the clinical data of 121 heart and respiratory failure patients who underwent ECMO treatment in Fu Wai Hospital from December 2004 to December 2009. Based on the different systems used, 121 patients were divided into two groups. In group M, there were 65 patients including 41 males and 24 females, with an age of 26.6±25.9 years; and they accepted Medtronic ECMO system treatment. In group Q, there were 56 patients including 39 males and 17 females, with an age of 32.4±23.9 years and they underwent Quadrox PLS ECMO system treatment. The evaluation of the clinical outcomes of these two different systems was based on the comparison of transmembrane pressure drop (TMPD), anticoagulation, hemolysis, plasma leakage, organ function, complication and hospital mortality between the two groups. Results Compared with group M, ECMO with Quadrox PLS system was associated with lower TMPD (at the beginning of ECMO: 15.0±6.0 mm Hg vs.28.0±5.0 mm Hg, P=0.000; 24 hours later: 16.0±5.0 mm Hg vs. 30.0±7.0 mm Hg, P=0.000) and less thrombus formation(7.1% vs.23.1%, P=0.037), less plasma leakage (0.0% vs. 50.8%, P=0.000), less hemolysis (14.3% vs. 29.2%, P=0.047). There were no significant differences between the two groups in support duration, complication rate, and hospital mortality(Pgt;0.05). Conclusion Both devices have similar effects for safe clinical application, but Quadrox PLS ECMO system has a relatively high biocompatibility with lower TMPD, less plasma leakage, and thrombus formation.
Healthcare-associated infection management has advanced rapidly in recent years. With the development of more standards and guidelines, infection control measures become more standardized and evidence-based. Evidence-based measures are increasingly applied in infection control, which promote more studies on the prevention and control of healthcare-associated infections. Furthermore, more new ideas of infection control have emerged, with old ones being challenged. The hand hygiene reform, multidrug-resistant organisms, and surgical site infections become the hot topics in recent years. In addition, whole-genome sequencing also provides more bases for understanding pathogen transmission in hospitals. Based on the high-quality studies published in recent years, this opinion review discusses these hot topics in the prevention and control of healthcare-associated infections.
ObjectiveTo summarize the method of quality management in long term video electroencephalogram (VEEG) monitoring process.MethodsTo summarize the VEEG monitoring process in 4 935 patients, the following methods were adopted: adequate preparation before examination, selection of suitable electrode wearing methods, regular inspection of the quality of the lead wire, inspection and observation of whether the electrodes have fallen off, process inspection, behavioral intervention guidance, timely manage the artifacts, pay more attention to the inducted experimental, timely identification of paroxysmal events, standardize the procedures for the management of seizures, standardize the processing of electrode cleaning and disinfection, continuously improve the quality.ResultsFour hundred and tworoy are paroxysmal events of various types occurred during the monitoring period. All of them were handled in time and the patients were all safe. Among these events, 4 children ended the examination in ahead of the normal procedure due to fever, crying or other reasons. two patients were transferred to intensive care unit due to changes in patients ’conditions such as hypopnea and decreased oxygen saturation of artery blood of finger. The remaining 4 829 patients completed VEEG detection for 8 ~ 24 h. and got good quality images.ConclusionsQuality management is a guarantee of qualified, high quality, low artifact EEG reports.
Objective To investigate the predictors for carbapenem-resistant Acinetobacter baumannii, Enterobacteriaceae and Pseudomonas aeruginosa (CR-AEP) as the pathogens of bloodstream infection (BSI) for intensive care unit (ICU) patients. Methods A retrospective case-control study based on ICU- healthcare-associated infection (HAI) research database was carried out. The patients who have been admitted to the central ICU between 2015 and 2019 in the ICU-HAI research database of West China Hospital of Sichuan University were selected. The included patients were divided into two groups, of which the patients with ICU-acquired BSI due to CR-AEP were the case group and the patients with BSI due to the pathogens other than CR-AEP were the control group. The clinical features of the two groups of patients were compared. Logistic regression model was used to identify the predictors of BSI due to CR-AEP.ResultsA total of 197 patients with BSI were included, including 83 cases in the case group and 114 cases in the control group. A total of 214 strains of pathogenic bacteria were isolated from the 197 BSI cases, including 86 CR-AEP strains. The results of multivariate logistic regression analysis showed that previous use of tigecycline [odds ratio (OR)=2.490, 95% confidence interval (CI) (1.141, 5.436), P=0.022] was associated with higher possibility for CR-AEP as the pathogens of BSI in ICU patients with BSI, while previous use of antipseudomonal penicillin [OR=0.497, 95%CI (0.256, 0.964), P=0.039] was associated with lower possibility for that. Conclusion Previous use of tigecycline or antipseudomonal penicillin is the predictor for CR-AEP as the pathogens of BSI in ICU patients with BSI.
ObjectiveTo investigate the effects of antimicrobial management program on inpatients outcomes and antimicrobial resistance among clinical isolates in a large-scaled university hospital.MethodsThe antibiotics use density (AUD) and antimicrobial resistance rate of multi-drug resistant bacteria before (from January 2009 to December 2010) and after (from January 2012 to December 2016) the intervention of antimicrobial management program in a large-scaled university hospital (4 300 beds) were calculated and compared, and the correlations of AUD with average length of hospital stay and mortality rate were analyzed.ResultsThe AUD was significantly decreased after intervention (P<0.001). The resistance rate of Staphylococcus aureus to oxacillin decreased (P<0.001). Among Gram-negative bacteria, the resistance rates to carbapenems in Acinetobacter baumannii (P<0.001) and Klebsiella pneumoniae increased (P=0.011). AUD was not correlated with the average length of hospital stay (P=0.644), while positively correlated with the in-hospital mortality rate (r=0.932, P=0.001).ConclusionsThe implementation of antimicrobial management program can significantly reduce the antimicrobial use and do not worsen patient outcomes in the hospital. The impact of the program on resistance varies significantly depending on both the bacterium and the agent, and carbapenem-non-susceptible Gram-negative bacilli emerges as a major threat. It is still necessary to combine other infection control measures.
ObjectiveTo systematically review the efficacy and safety of major ozonated autohemotherapy in the treatment of ischemic stroke.MethodsPubMed, EMbase, The Cochrane Library, Web of Science, CBM, WanFang Data, VIP and CNKI were electronically searched to collect randomized controlled trials (RCTs) of the efficacy and safety of major ozonated autohemotherapy in the treatment of ischemic stroke from inception to July 1st, 2020. Two reviewers independently screened literature, extracted data and assessed risk of bias of included studies. Meta-analysis was then performed by using RevMan 5.3 software.ResultsA total of 25 RCTs involving 3 681 patients were included. The results of meta-analysis showed that the major ozonated autohemotherapy combined with conventional therapy in the treatment of patients with ischemic stroke in terms of total effective rate (RR=1.20, 95%CI 1.15 to 1.25, P<0.001), national institutes of health stroke scale (MD=−3.15, 95%CI −4.72 to −1.59, P<0.001), total cholesterol (MD=−1.00, 95%CI −1.48 to −0.53, P<0.001), triglyceride (MD=−0.74, 95%CI −1.04 to −0.43, P<0.001), low-density lipoprotein cholesterol (MD=−0.65, 95%CI −1.22 to −0.09, P=0.02), and activity of daily living (MD=11.97, 95%CI 4.48 to 19.47, P=0.002) were superior to the conventional treatment group. There was no significant difference between the two groups in high-density lipoprotein cholesterol (MD=0.25, 95%CI −0.46 to 0.96, P=0.49) and the incidence of adverse effects (OR=3.15, 95%CI 0.93 to 10.63, P=0.06).ConclusionsThe major ozonated autohemotherapy can significantly improve the prognosis of patients with ischemic stroke while not affecting the adverse effects. Due to the limited quality and quantity of the included studies, more high-quality studies are needed to verify the above conclusions.
