  • Basic Theory and Methods and Application of Onehealth Software for Evidence-based Decision-making

    Onehealth, an evidence-based decision-making software, is based on the United Nations' epidemiological reference modules to predict the effect of health services. Onehealth is a large database. The software is using activitybased costing, simulating investment costs of health system and changes of mortality in different coverage levels. By the cost of inputs/avoid deaths, it could quantify the cost of health services effectiveness and provide an intuitive basis for the rational allocation of health resources. This study introduces the relevant concepts, model structures and applications of Onehealth. We took the study of child nutrition interventions in Sudan for example and to present Onehealth tool's operating. As a new auxiliary and evidence-based decision-making software with scientific and rigorous theoretical approach, Onehealth has practical significance on the national or regional macro decision-making.

  • How to Perform Network Meta-Analysis of Continuous Variable Using R, GeMTC and Stata softwares

    The published methodological studies about network meta-analysis mostly focused on the binary variables, but study focused on the continuous variables was few. This study introduces how to use R, GeMTC and Stata softwares jointly to produce various graphics of continuous variable network meta-analysis. It also introduces how to perform the convergence diagnostics, trace and density plot, forest, rank probabilities and rankogram, internal relationship summary chart, network plot, contribution plot and publication bias test.

  • Lanthanum Carbonate in the Treatment of Chronic Kidney Disease with Hyperphosphatemia: An Overview of Systematic Reviews

    ObjectivesTo evaluate the methodological bias and the reliability of the conclusions of systematic reviews (SRs) of lanthanum carbonate in the treatment of chronic kidney disease with hyperphosphatemia. MethodsWe electronically searched databases including PubMed, EMbase, The Cochrane Library, PROSPERO, CNKI, CBM, WanFang Data and VIP to collect systematic reviews and meta-analysis about lanthanum carbonate in the treatment of chronic kidney disease with hyperphosphatemia from inception to August 31st, 2016. Two reviewers independently screened literature and extracted data, then AMSTAR tool was used to assess the methodological quality of included studies and the GRADE tool was used to grade the evidence quality of outcome measures included in the SRs. ResultsA total of eight relevant SRs were included and containing three main outcome measures. The assessment results of AMSTAR tool suggested that:four SRs were of high quality, and the other four were of medium quality. GRADE results showed:for serum phosphorus level, compared with placebo, the quality of the evidence of three SRs were medium, low and very low; compared with calcium carbonate or conventional phosphorus binder, four SRs were low, low, low and very low; compared with sevelamer, one SR was low. For serum calcium level, compared with placebo, the quality of the evidence of three SRs were high, medium and low, respectively; compared with calcium carbonate or conventional phosphorus binder, five SRs were low, low, low, very low and very low; compared with sevelamer, one SR was very low. For serum iPTH level, compared with placebo, the quality of the evidence of three SRs were medium, low and very low; compared with calcium carbonate or conventional phosphorus binder, five SRs were medium, low, low, very low and very low; compared with sevelamer, one SR was low. ConclusionAt present, methodological quality assessment for the treatment of hyperphosphatemia in chronic kidney disease with lanthanum carbonate is generally not high and the level of evidence for the conclusion is generally low. In drug safety, especially in the occurrence of adverse events of the digestive system is still controversial, and a large amount of high quality experimental is needed to demonstrate the safety of its long-term use. Clinicians need to be cautious in using these evidence to make clinical decisions.

  • Helical Tomotherapy for Cancer Treatment: A Rapid Health Technology AssessmentΔ

    ObjectiveTo rapidly review the effectiveness, safety, costs and applicability of helical tomotherapy (HT), so as to provide currently-available best evidence for decision makers of government health policies. MethodsWe electronically searched databases including PubMed, EMbase, The Cochrane Library, CNKI, WanFang Data, VIP, CBM and other relevant professional websites. Two reviewers independently screened literature according to the inclusion and exclusion criteria, extracted data, assessed quality, and then performed descriptive analysis. Resultsa) We finally included 150 studies, encompassing 5 health technology assessments (HTAs), 18 clinical controlled trials (CCTs), and 127 observational studies. b) Four included HTAs were published during 2006-2009, providing fairly less evidence of low quality and the results of 145 primary studies showed that:HT was mainly used in the treatments of 14 types of cancer, with relatively low total toxicity and high survival rates. Although the quality of the included studies was poor, there was much evidence about prostate cancer, head and neck cancer, nasopharynx cancer, cervical cancer, lung cancer and liver cancer, with accumulatively enough sample size and fairly reliable results in HT's efficacy and safety. And c) a total of 56 clinical trials had bee registered in Clinicaltrials.gov, most of which were registered by the occident; among them, 9 had been come to the end, yet without results published. ConclusionCurrent evidence of this study showed that, HT is safe and effective in clinic. But the abovementioned conclusion needs to be verified by conducting more high quality studies with long-term follow-up. The costs of HT in procurement, maintenance and application are high; and the skills, training and qualification of operators are required. We suggest that the procurement of HT should be reduced; it should be allocated rationally and effectively used after comprehensive assessment in China's cancer epidemiology characteristics, health resource allocation, disease burden, medical service level, etc.; and also high quality studies with long-term follow-up should be financially supported on the basis of establishing projects, so as to provide local evidence and consistently guide and improve scientific decision making.
