【摘要】 目的 研究综合医院心理咨询初诊患者的特征。 方法 对2007年1月1日-2009年12月31日初诊患者的性别、年龄、居住地、学历、职业、咨询目的与疾病分布等特征进行统计分析。 结果 共纳入1 933例初诊患者,女性略多于男性(男女比1∶1.1);年龄3~75岁,以青少年(10~19岁,34.5%)和青年(20~29岁,29.7%)为主;当地居民居多(66.0%),外地居民亦占较大比例(34.0%);学历主要分布在中学(初中:21.5%;高中:35.6%)和大学(33.6%);职业分布以学生比例最大(40.0%),其次为无固定职业者(14.9%),专业技术人员(12.5%),商业、服务人员(9.4%),办事人员和有关人员(7.7%)。患者中有心理问题的健康人占27.5%,以亲子教育咨询(53.4%)和婚恋咨询(41.5%)为主;患精神障碍的患者占72.5%,以心境障碍(30.6%)、神经症(23.1%)和精神分裂症(10.1%)为主,新确立的精神障碍网络成瘾占1.1%。 结论 综合医院心理咨询初诊患者来源广泛,需要更加关注青少年、学生和无固定职业者的心理问题。【Abstract】 Objective To investigate the characteristics of the new clients from the psychological counseling clinic in general hospital. Methods The clients’ data were prospectively analyzed from the successively registered database of the counseling clinic from January 1, 2007 to December 31, 2009. The data included sex, age, habitation, educational background, career, problems, and diagnosis of the new clients. Results A total of 1 933 clients were included in the study, and the sex ratio was 1∶1.1(male∶female). The age ranged from three to 75 years old. The clients aging from 10 to 29 yeas old accounted for 64.2%. Sixty-six percent of the clients were local residents. The education backgrounds of them were middle school (21.5%), high school (35.6%), and graduate (33.6%). Forty percent of the clients were students and 14.9% had no regular works. Special technician, business and service workers, and office workers accounted for 12.5%, 9.4%, and 7.7%, respectively. The percentage of healthy clients was 27.5%, whose main problems were child education (53.4%), and marriage relationship (41.5%). And 72.5% of the clients suffered from mental disorders, with mood disorder (30.6%), neurosis (23.1%), and schizophrenia (10.1%). Internet addiction, as a new established mental disorder in China, accounted for 1.1%. Conclusion At present, the clients of counseling clinic in general hospital are more widespread, and more attention should be paid to the young, the students, and the people without regular work.
ObjectivesTo provide methodology support for the tracking assessment of specific resolution execution through evidence-based construction of tracking assessment index system for resolution execution of WHO essential medicine accessibility resolution, so as to further promote and improve the establishment of national essential medicine policies, and offer a methodology reference to survey and assess the resolution executions in other public health fields.MethodsA multi-disciplinary team was set up to preliminarily construct the index system by means of earlier system assessment index through index screening based on Delphi method. The weight of each index was determined by analytic hierarchy process.ResultsAfter two rounds of expert consultation, the index system available for resolution execution measures and results of WHO essential medicine accessibility were established, including 9 indexes of resolution execution measures. Meanwhile, the execution results of index contained 4 first class indexes, 13 second class indexes and 36 third class indexes. Each of the indexes obtained its own weight according to degree of importance.ConclusionsIn this study, the assessment index for resolution execution of essential medicine accessibility is established, however, the empirical research is still required to further verify the scientificity as well as feasibility of this index system.
ObjectivesTo initially construct a scientific, reasonable and precision medicine technology value judgment framework suitable for China’s national conditions based on expert consultation method, so as to provide scientific value judgment system support for China's medical insurance decision-making.MethodsThe preliminary evaluation indicator system for precision medicine technology value was established by using literature analysis and expert consultation method, and the direct weighting method was used to determine the indicators weight.ResultsAfter two rounds of expert consultation, an indicator system suitable for the value judgment of precision medicine technology in China was constructed, including 5 primary indicators (health needs, health effects, economics, innovation and suitability) and 14 secondary indicators. Each indicator was weighted according to importance.ConclusionsA set of precision medicine value judgment indicator system suitable for China has been initially established, which lays a certain foundation for further measurement research of the indicator system and provides a scientific basis for medical insurance decision-making.
ObjectiveTo investigate the job satisfaction, emotional state and related factors of medical staff participating in online consultation of West China Internet Hospital during the COVID-19 epidemic.MethodsThrough literature review and expert consultation (Delphi method), the questionnaire was developed, and the online consulting medical staff of West China Hospital of Sichuan University were invited to conduct the questionnaire survey from 26 January to 19 June 2020, and finally the statistical analysis was summarized.ResultsA total of 132 valid questionnaires were retrieved. Of the 132 subjects, 127 people (96.2%) expressed satisfaction or special satisfaction with the online consulting office format; 103 respondents (78.0%) said that online consulting did not affect or completely did not affect the work and life; 81 people (61.4%) consulted online more than 5 days a week, and 108 people (81.8%) worked within 2 hours a day; the vast majority (97.7%) of the research subjects were satisfied with the content of the training materials and the related support work of the coordination group. Only 29 (22.0%) of the study participants believed that the epidemic caused negative emotions, mainly due to the severity of the epidemic.ConclusionThe online consulting medical staff are satisfied with the office form, training materials and coordination work group of the COVID-19 epidemic, and think that it does not affect their work and life. 22.0% of medical staff have negative emotions, and the severity of the epidemic is the main reason.
ObjectivesTo use the Delphi method to preliminarily perfect the comprehensive evaluation index system which had been initially constructed by systematic review processes of diabetes food for special medical purposes (FSMP).MethodsThree rounds of expert consultation by Delphi method were compared to define indices held in the evidence-based comprehensive evaluation index system of diabetes FSMP and ways to distinguish effects.ResultsThe preliminary perfect index system after 3 rounds of expert consultation contained 4 primary directory indicators, 15 secondary directory indicators, and 20 tertiary indicators. Among them, the end index which could directly be used for evaluation contained a total of 27 indicators.ConclusionsThe index system is categorized into 4 segments of parameters, which are medical, nutritional, safety and other indices, which has addressed the problem posed by CFDA documents on how to scientifically validate the medical effect and safety of FSMP. It provides great significance to assess and supervise FSMP prior to and after it goes to public, assess characteristics and advantages of MNT, and improves nutrition economy and its social benefits.
