  • Value of MRI and MDCT in Detecting Both Inferior Vena Cava Tumor Thrombus and Vena Cava Wall Invasion in Renal Cell Carcinoma: A Systematic Review

    Objective To evaluate the value of MRI and MDCT in detecting both inferior vena cava tumor thrombus and vena cava wall invasion in renal cell carcinoma. Methods Databases including PubMed, EMbase, The Cochrane Library, MEDLINE (Ovid), CBM, CNKI, VIP and WanFang Data were searched from January 2000 to February 2012. Relevant studies were screened on the basis of the inclusion and exclusion criteria, and then quality assessment and data extraction were conducted. Then heterogeneity test and meta-analysis were conducted using RevMan 5 and Meta-disc 1.4. Results A total of 6 trials involving 244 patients and 246 cases of renal cell carcinoma were included. The results of meta-analysis showed that, for the MRI group and the MDCT group, the sensitivity was 0.963 and 0.952, the specificity was 0.969 and 0.979, the value of +LR was 9.759 and 15.57, the value of −LR was 0.091 and 0.108, and the dOR was 198.71 and 251.54, respectively. There were no significant differences in pooled effect-size among groups (Pgt;0.05). The area under curve (AUC) of summary ROC curve analysis as well as Q index of the MDCT group were 0.981 8 and 0.940 7, respectively. Conclusion There is no significant difference in the value of MRI and MDCT in detecting inferior vena cava tumor thrombus induced by renal cell carcinoma. More original studies on vena cava wall invasion by tumor thrombus should be conducted in the future due to the limitation of current materials.

  • Effectiveness and Safety of Flexible Ureteroscope Lithtripsy and Percutaneous Nephrolithotomy for Renal Calculus: A Systematic Review

    Objective To systematically review the effectiveness and safety of flexible ureteroscope lithtripsy (fURL) and percutaneous nephrolithotomy (PCNL) in treating renal calculus. Methods Such databases as MEDLINE, EMbase, The Cochrane Library, CNKI, CBM, VIP, and WanFang Data from January, 1990 to August, 2012 were searched to comprehensively collect the clinical trials that compared fURL and PCNL in treating renal calculus. Two reviewers independently screened studies according to exclusion and inclusion criteria, extracted data, and assessed the methodological quality. Then, meta-analysis was performed using RevMan 5.1 software. Results Eight non-randomized controlled trials involving 536 patients were included. The results of meta-analysis showed that, PCNL was better than fURL in stone clearance (OR=0.26, 95%CI 0.15 to 0.46), but fURL was better than PCNL in postoperative pyrexia (OR=0.1, 95%CI 0.42 to 3.35), the incidence of blood transfusion (OR=0.17, 95%CI 0.03 to 1.00), and the duration of hospitalization (P=0.45, I2=0%). Conclusion Current evidence has proved that PCNL is better than fURL in decreasing stone clearance, fURL is better than PCNL in complication, the duration of hospitalization, and medical costs.

  • Change of Renal Endothelin1 Excretion and Its Relation to Renal Dysfunctions in Bile Duct Ligated Rats

    ObjectiveTo investigate the change of renal endothelin (ET) excretion and its relation to renal dysfunctions in obstructive jaundice.MethodsSixty male Wistar rats were randomized into two groups, the common bile ducts were ligated to establish the model of obstructive jaundice in experimental group, and only sham operation was done in control group. Ten rats were taken from each group at 5, 10 and 15 days respectively after operation, renal functions were evaluated by paminohippuric acid clearance (CPAH), inulin clearance (CIN) and fractional sodium excretion (FENa+); furthermore, plasma endotoxin (EX) level was determined, and ET1 contents in renal arterial plasma, renal venous plasma and renal tissue were detected. ResultsOnly FENa+ was significantly increased at the 5th day in experimental group; since the 10th day, all the three renal functional parameters gradually decreased, and FENa+ was significantly lower than that in control group at 15th day (P<0.01 vs control). ②The plasma EX sustained at significantly higher levels after operation in experimental group (P<0.01 vs control). ③The renal arterial plasma ET1 was significantly decreased, while the contents in renal venous plasma and renal tissue were significantly increased after operation in experimental group (P<0.01 vs control). ④There were positive correlation between plasma EX and renal ET1 content, negative correlation between renal ET1 content and CPAH/CIN, and positive correlation between renal ET1 content and FENa+ (P<0.01).ConclusionThe increased excretion of renal ET stimulated by endotoxemia may play an important role in the renal dysfunctions in obstructive jaundice.

  • Measurement and Significance of Serum Nitric Oxide Level in Patient with Obstructive Jaundice and Renal Dysfunction

    ObjectiveTo study the change and significance of the serum nitric oxide (NO) level in patient with obstructive jaundice complicated with renal dysfunction. MethodsThe level of NO, BUN, Cr in serum and the activity of NOS in 25 patients with obstructive jaundice and renal dysfunction and 26 healthy adults was studied.ResultsThe patients’ serum NO level and the activity of NOS were significantly lower than those in the control group(P<0.01),whereas the serum BUN and Cr levels were significantly higher than those in control group(P<0.01). The linear correlation analysis showed that the serum NO had a negative correlation between serum BUN and Cr level (P<0.01). ConclusionThe patients with obstructive jaundice and renal dysfunction may lead to the decrease of serum NO level. NO may have some protective effects to the renal function during obstructive jaundice.

  • THE ROLE OF NITRIC OXIDE IN THE PATHOGENESIS OF RENAL DYSFUNCTION DURING ACUTE BILIARY INFECTION IN RATS

    Objective To investigate the effect of nitric oxide (NO) on the renal function in acute biliary infection. Methods 35 Wistar rats were divided randomly into acute biliary infection group (AC), group of AC with Larginine(L), group of L-NAME(N), group of simple biliary obstruction(O) and a sham-operated group(SO), and serum NO、 BUN、 Cr and NOS were determined and pathologic changes of liver and kidney were observed. Results NO and NOS were significantly higher in L group than in any other groups (P<0.05), BUN and Cr were significantly lower than in AC and N groups (P<0.05), but showed no significance compared with O group (P>0.05). The pathology of kidney in L group showed a less severe change than that in AC group; NO and NOS in N group were lower than those in other groups. BUN and Cr were higher than those in other groups (P<0.05). Conclusion NO has a protection for renal function in acute biliary infected rats the mechanism being related to its dilative effect on renal vessels and increased renal blood perfusion.

  • Clinical analysis of 29 cases with pneumonia in renal transplant recipients and literature review

    Objective To investigate the clinical features of pnuemonia in renal transplant recipients in order to improve the clinical diagnostic and treatment efficacy.Methods The clinical data of 29 recipients with pneumonia following renal transplantation in Peking University People’s Hospital from January 1,1999 to July 31,2006 were collected and analyzed retrospectively.Results Of the 29 cases with pneumonia,one case(3.4%,1/29)were diagnosed as nosocomial acquired pneumonia and twenty eight cases(96.6%,28/29)as community acquired pneumonia.Of the total,cytomegalovirus (CMV) pneumonia were validated in 19 cases, bacteria pneumonia in 10 cases, fungus pneumonia in 3 cases,and Pneumocystis Carini infection in one case while none pathogen were confirmed in 5 cases with pneumonia.37.5%(9/24)cases with pneumonia which pathogen were ascertained were resulted from multiple pathogen infection.Opportunistic organism is the main pathogen of pneumonia in renal transplant recipients and the disease commonly manifested as interstitial pneumonia. 27 cases(93.1%)developed to severe pneumonia in which 15 cases were deteriorated to acute respiratory distress syndrome.Excluding 5 cases who withdrew from the hospital,4 cases (16.7%,4/24)died and 20(83.3%,20/24) cases were cured.During the second to the sixth month especially the second to the thrid month after renal transplantation the recipients were at higher-risk of pneumonia.Conclusions The majority of pneumonia in kidney recipients were severe community acquired pneumonia caused by multiple pathogens.CMV pneumonia and bacteria pneumonia are the most common types and major causes of mortality.Mortality of fungus pneumonia and CMV pneumonia are highest.Proper diagnostic procedures and therapeutic strategies are critical to improve survival rate.

  • Clinical study of maintenance factors in metabolic alkalosis of critically ill patients during perioperative perio

    Objectives To study the role of the kidney in the maintenance of metabolic alkalosis of critically ill patients during perioperative period.Methods The patients who had metabolic alkalosis in the surgical intensive care unite(SICU) from Nov 2004 to Feb 2005 were enrolled in the alkalosis group;and the control group were the perioperative patients in the department of hepatic surgery at the same time,those who had acid-base imbalance were excluded.The enrolled patients underwent routine tests and some parameters such as creatinine clearance rate(Ccr,to evaluate glomerular filtration rate),titratible acid,ammonium ion,urinary bicarbonate,net acid excretion were calculated.Results The Ccr of the alkalosis group and control group was(76.2±37.1)mL/min vs(98.5±31.9)mL/min,respectively(P=0.042) with a decrement of 22% in the alkalosis group.The titratible acid was(25.2±19.4)mmol/24 h vs(49.9±26.4)mmol/24 h,respectively(P=0.002);the net acid excretion was(156.5±84.3) mmol/24 h vs(117.5±32.1)mmol/24 h,respectively(P=0.047);the ammonium ion was(140.6±81.6) mmol/24 h vs(78.7±16.3)mmol/24 h,respectively(P=0.002).The postoperative electrolytes of the alkalosis group and control group:[K+] was(3.51±0.67)mmol/L vs(4.14±0.59)mmol/L,respectively(P=0.002);[Cl-] was(98.4±8.3)mmol/L vs(102.8±3.0)mmol/L,respectively(P=0.035);[Ca2+] was(2.14±0.21)mmol/L vs(2.25±0.14)mmol/L,respectively(P=0.049);[P] was(0.83±0.34)mmol/L vs(1.11±0.23)mmol/L,respectively(P=0.004);[Na+] was(139.6±7.7)mmol/L vs(140.8±4.6)mmol/L,respectively(P=0.535);[Mg2+] was(0.94±0.15)mmol/L vs(0.90±0.16)mmol/L,respectively(P=0.338).Conclusions Decreased glomerular filtration rate and enhanced renal acidification function are the important factors that maintain the metabolic alkalosis during perioperative period.Potassium,chloride,calcium and phosphorus are decreased during metabolic alkalosis,while sodium and magnesium has no significant change.

  • Comparison of Accuracy of Different Renal Function Measurements for Predicting Adverse Events after Off-pump Coronary Artery Bypass Grafting

    Objective To compare the accuracy of different renal function measurements for predicting postoperativeadverse events after off-pump coronary artery bypass grafting (OPCAB) for Chinese patients. Methods Clinical data of 283 Chinese patients undergoing isolated OPCAB from January 2010 to December 2011 in the First Hospital of Peking University were retrospectively analyzed. There were 194 male and 89 female patients with their age of 65.0±9.7 years. Estimated glomerular filtration rate (eGFR) was calculated using Cockcroft-Gault equation,Modification of Diet in Renal Disease (MDRD) study equation and Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) equation respectively.Logistic multivariate regression analysis was performed to compare the accuracy of these 3 different renal function measure-ments for predicting postoperative adverse events. Results Logistic multivariate regression analysis showed that preoper-ative renal dysfunction was an independent risk factor for higher postoperative morbidity,more blood transfusion,prolongedhospitalization,mechanical ventilation time and length of ICU stay. Among the different postoperative complications,preop-erative renal dysfunction was an independent risk factor for postoperative acute renal injury,gastrointestinal bleeding,new onset atrial fibrillation and low cardiac output syndrome. Among Cockcroft-Gault equation,MDRD study equationand CKD-EPI equation to calculate eGFR,CKD-EPI equation was most accurate to predict postoperative morbidity (OR=1.227),acute renal injury (OR=1.534),new onset atrial fibrillation (OR=1.184),prolonged hospitalization(OR=1.160),mechanical ventilation time (OR=1.165) and ICU stay (OR=1.151). Conclusion Preoperative renal dysfunction is an independent risk factor for postoperative adverse events after OPCAB. CKD-EPI equation is more suitable for predicting postoperative adverse events after OPCAB for Chinese patients.

  • Impact of Preoperative Renal Dysfunction on Outcomes of Cardiac Surgery and Risk Factor Analysis

    Objective To investigate the prognosis and risk factors of cardiac surgical patients with preoperative renal dysfunction,and evaluate the accuracy of estimated glomerular filtration rate (eGFR) as a predictor of adverse outcomes. Methods A total of 2 151 adult patients undergoing cardiac surgery in Renji Hospital,School of Medicine of Shanghai Jiaotong University from January 2005 to December 2009 were included in this study. There were 1 267 male patientsand 884 female patients with their average age of 58.7 (18-99)years. Clinical characteristics of patients with preoperative renal dysfunction,severity of postoperative acute kidney injury (AKI)and patients’ outcomes were analyzed. Multivariate logistic regression was performed to analyze perioperative risk factors of postoperative AKI. Receiver operating characteristic(ROC) curve was used to evaluate the accuracy of eGFR to predict patients undergoing postoperative renal replacement therapy (RRT) and in-hospital death. Results A total of 221 patients (10.27%) had preoperative renal dysfunction,among whom 124 patients (56.11%) developed postoperative AKI. Patients with preoperative renal dysfunction were older,had more comorbidities including hypertension and diabetes mellitus,were more likely to receive postoperative RRT,and had worse outcomes. Patients with decreased preoperative eGFR had significantly higher in-hospital mortality. Patients with preoperative renal dysfunction who developed postoperative AKI had the worst prognosis. Multivariate logistic regression showed that hypertension (OR=4.497,P=0.003),postoperative central venous pressure (CVP) <6 cm H2O (OR=16.410,P=0.000) and postoperative CVP>14 cm H2O (OR= 5.178,P=0.013) were independent predictors of postoperative AKI for patients with preoperative renal dysfunction. The areas under the ROC curves of eGFR to predict in-hospital mortality and postoperative RRT were 0.691 and 0.704 respectively (95% CI 0.630-0.752,P=0.000;95% CI 0.614-0.795,P=0.001). Conclusion Patients with preoperative renal dysfunction are older,have more comorbidities,higher likelihood to develop postoperative AKI and worse prognosis. Hypertension,postoperative CVP<6 cm H2O and postoperative CVP>14 cm H2O are independent predictors of postoperative AKI for patients with preoperative renal dysfunction. We believe eGFR can accurately predict the risk of adverse kidney outcomes and in-hospital death of patients undergoing cardiac surgery.

  • Protective Effects of Ulinastatin on the Peri-operative Liver and Renal Function in Patients Undergoing Cardiac Surgery for Tetralogy of Fallot

    Abstract:  Objective To evaluate the protective effects of Ulinastatin on the peri-operative liver and renal function in patients undergoing cardiac surgery for tetralogy of Fallot (TO F).  Methods Thirty-eight patients with TOF were divided into Ulinastatin group and control group according to admission sequence, 19 cases in each group.For Ulinastatin group, intravenous Ulinastatin was given with a dosage of 10 000U /kg at 1h before operation, 1h and 24 h after operation. For control group, no Ulinastatin was given. 10 ml fresh urine and 2 ml blood samples were collected before operation, and postoperative 1h, 10h, 24h, 48h and 72h, respect ively. The liver and renal functions were measured. Fluid intake, urine output, chest drainage, dosage of furosemide, durations of mechanical ventilation and intensive care unit ( ICU ) stay were recorded.  Results Neither arrhythmia nor low cardiac output syndrome occurred for both groups. No peri-operative death. Compared with control group, dose of furosemide, period of mechanical ventilation were lower, while urine output was higher in Ulinastat in group; the aberrant climax value of urine pro tein and N-acetylglucosam inidase (NAG) were lower in Ulinastatin group (10h post-operat ively, urinem icroalbum in: 65. 2 ± 58. 3mg/L vs. 71. 8 ±58. 9mg/L ; urine transferrin: 5. 8 ± 3. 6mg/L vs. 7. 4 ± 5. 4mg/L ; urine immunoglobulin G: 26. 9±20. 3mg/L vs. 31. 3±23. 3mg/L ; 1h post-operat ively; urine NAG: 61. 4±81. 6U /L vs. 76.1±48. 5 U /L ; P lt; 0. 05) and maintained in shorter period (P lt; 0. 05) , it returned to baseline value at 48h and 72 h post-operatively. The value of alanine aminotransferase (ALT) significantly increased post-operatively at every time points in control group (P lt; 0. 01) , w hile no obvious change in Ulinastat in group (P gt; 0. 05). The increased value of aspartate aminotransferase (AST ) in Ulinastatin group was significantly lower than that in control group (10h post-operat ively: 144. 4±20. 8U /L vs. 202. 7±74. 1U /L ; P lt; 0. 01). The value of AST returned to baseline value at 48h and 72h post-operat ively.  Conclusion  U linastatin is an effect ive strategy for protecting peri-operat ive liver and renal function of the patients with tetralogy of Fallot and the clinical application of Ulinastatin is safe and effective.
