ObjectiveTo summarize research progress of comprehensive treatment based on gene therapy, immunotherapy, and targeted therapy in recent years in order to improve understanding and treatment level of triple negative breast cancer (TNBC).MethodThe literatures about TNBC treatment in recent 5 years were reviewed and summarized.ResultsTNBC was more invasive than other types of breast cancer due to its lack of targeted receptors, and its recurrence and metastasis were earlier. The treatment plan was still mainly surgical treatment, supplemented by the chemotherapy and radiotherapy.ConclusionsAlthough recent studies of TNBC in surgical treatment, chemoradiotherapy, targeted therapy and other aspects have shown a good clinical application prospect, more evidences of clinical trials with large samples are still needed. With the deepening of molecular mechanism research, endocrine therapy and targeted drug therapy, including androgen-receptor-positive, have provided some new ideas for treatment of TNBC.
ObjectiveTo explore the expression of collagen Ⅳ in breast cancer and its clinical significance. We analyzed the correlation of the results with other prognostic parameters which included tumor size, status of estrogen receptor, axillary nodal status, TNM grade, and 5 years survival. The expression of collagen Ⅳ in 93 cases of human primary breast cancer as well as 5 cases of benign breast masses were examined.MethodsUsing monoclonal antibodies of collagen Ⅳ, the expression of collagen Ⅳ in breast masses were detected with immunohistochemical technique (LSAB).ResultsThe absent expression of collagen Ⅳ in the tumor masses was correlated with axillary lymph node involvement, tumor size and poor prognosis (5 years survival). The patients who had no expression of collagen Ⅳ in tumor masses had a shorter survival. ConclusionThe expression of collagen Ⅳ in tumor samples are correlated with axillary node involvement and prognosis. Collagen Ⅳ would be helpful for evaluation of invasion and treatment in breast carcinoma.
目的 探讨上腹部手术后腹腔镜胆囊切除术(LC)的可能性及手术方法与技巧。方法 回顾性分析我院2005~2009年期间对有上腹部手术史行LC的23例患者的临床资料。结果 23例患者中慢性结石性胆囊炎18例,胆囊息肉5例。既往均有上腹部手术史,其中胃大部切除术后19例,胃平滑肌瘤切除术后2例,脾破裂修补术后2例。采用闭合法穿刺建立气腹,分离粘连,暴露胆囊全貌及Calot三角,顺行或逆行切除胆囊。23例中LC成功21例; 因粘连致密,胆囊管无法辨认,中转开腹2例。手术时间45~140 min,平均67 min。全组无明显出血、内脏损伤、胆管损伤、胆汁漏等并发症发生。结论 部分上腹部手术后胆囊良性疾病行LC术可行。
ObjectiveTo construct eukaryotic expression vector of pEGFP-N3-TFPI-2, and to provide the base of studying the function of TFPI-2 gene. MethodsExtraction of total RNA from placental tissue was extracted at first, and then reverse transcriptase synthesis of cDNA was carried out. The cDNA fragment of TFPI-2 gene which was obtained by real time PCR (RT-PCR) was inserted into eukaryotic expression vector of pEGFP-N3. After double digestion with XhoⅠand KpnⅠ, the recombinant vector of pEGFP-N3-TFPI-2 was identified in 1% agarose gel electrophoresis and was tested by the sequence analysis. Then, the recombinant vector of pEGFP-N3-TFPI-2 (transfection group) and vector of pEGFP-N3 (blank control group) were transfected into Top10 competent cells with LipofectamineTM 2000, but no transfection-related treatment was performed in cells of untransfection group. Western blot method was used to test the expression of TFPI-2 protein in cells of 3 groups. ResultsThe purity of total RNA which were analysis by agarose gel electrophoresis and spectrophotometry were fit for PCR. After coding of TFPI-2 gene fragment and eukaryotic expression vector of pEGFP-N3, the recombinant plasmid of pEGFP-N3-TFPI-2 were got double digestion with XhoⅠand KpnⅠ, and was identified in 1% agarose gel electrophoresis, of which showing that there were 2 specific amplification of strips at 708 bp and 4 700 bp. Result of sequence analysis confirmed that the size of recombinant vector was consistent with the theoretical value. Results of Western blot showed that the expression of TFPI-2 protein in transfection group (0.657 3±0.032 5) was higher than those of blank control group (0.301 7±0.028 7) and untransfection group (0.314 3±0.026 6), P < 0.01. ConclusionsThe eukaryotic expression vector of pEGFP-N3-TFPI-2 has been constructed successfully, which laiding the foundation for the analysis about function of TFPI-2 gene.
目的 观察糖皮质激素对胎盘组织促肾上腺皮质激素释放激素(CRH)的分泌水平的影响。 方法 收集2006年1月-3月住院分娩的正常妊娠妇女的胎盘组织与妊娠肝内胆汁淤积症(ICP)患者胎盘及其血清各10例。分3组进行胎盘组织培养,即正常胎盘组、ICP胎盘组,正常胎盘组织加ICP患者血清组,分别用放射免疫法测定各组加与不加地塞米松胎盘组织培养液中CRH的水平。 结果 正常组与正常胎盘加地塞米松组培养24、48、72、96 h其CRH分泌水平分别为:(74.81 ± 27.92)、(63.71 ± 24.72)、(91.87 ± 41.64)、(98.90 ± 42.52) pg/mL;(66.94 ± 29.62)、(77.39 ± 31.84)、(61.89 ± 33.94)、(75.13 ± 36.98) pg/mL,两组比较差异有统计学意义(P>0.05)。ICP组与ICP加地塞米松组培养上清液中CRH水平在24、48、72、96 h其CRH分泌水平分别为:(48.28 ± 16.56)、(60.20 ± 29.97)、(72.92 ± 31.65)、(69.22 ± 29.33)pg/mL;(41.81 ± 25.00)、(57.36 ± 39.75)、(57.72 ± 23.29)、(61.43 ± 20.77)pg/mL, 两组比较差异有统计学意义(P>0.05);正常胎盘加ICP血清培养组与正常胎盘加ICP血清加地塞米松培养组上清液中CRH水平在24、48、72、96 h其CRH分泌水平分别为:(84.9 ± 34.98)、(74.5 ± 29.93)、(71.1 ± 27.26)、(81.0 ± 37.18)pg/mL;(76.29 ± 33.11)、(63.70 ± 24.20)、(64.85 ± 28.39)、(67.65 ± 33.20)pg/mL,两组比较差异有统计学意义(P>0.05)。3组加入地塞米松培养的胎盘组织,CRH分泌水平并无明显改变。 结论 地塞米松不影响体外培养胎盘组织CRH分泌。
Objective To explore the tumor shrinking pattern, the image evaluation methods of the residual lesions after neoadjuvant chemotherapy, and the indications, the resection range, the surgical margins, the management of sentinel lymph node biopsy (SLNB) and axillary lymph nodes of breast conserving surgery after neoadjuvant chemotherapy. Methods Domestic and foreign literatures about the breast conserving surgery after neoadjuvant chemotherapy were collected and reviwed. Results Breast conserving surgery after neoadjuvant chemotherapy raised the rate of breast conserving surgery and improved the postoperative life quality of patients, while the overall survival rate had no significant difference compared with mastectomy. Tumor shrinking pattern and its related factors after neoadjuvant chemotherapy remain required further study. At present, the indications for breast conserving surgery after neoadjuvant chemotherapy still mainly refered to that for early stage breast cancer. Resection range was more recognized to the scope of residual tumor lesions after chemotherapy. The margins were more widely accepted as which were without tumor involved. Although there were some controversies about the use of SLNB and the management of axillary lymph nodes based on the results of SLNB in breast conserving surgery after neoadjuvant chemotherapy, it was still considered necessary at the basis of the accurate biopsy technique acquired. Conclusions At present, breast conserving treatment after neoadjuvant chemotherapy is considered safe and effective. However, it is necessary to proceed cautiously under the conditions of adequate communication of local recurrence rate, standardized local treatment, standard postoperative radiotherapy, systemic adjuvant therapy, and regular follow-up.
In November 2022, the International Liaison Committee on Resuscitation updated the International Consensus on Cardiopulmonary Resuscitation and Emergency Cardiovascular Care Science with Treatment Recommendations for the sixth time. The 2022 review includes 21 topics addressed with systematic reviews by the Recovery Task Force of International Liaison Committee on Resuscitation. Among them, there are nine topics related to life support for newborns and children, including public-access defibrillation devices for children, pediatric early warning systems, maintaining normal temperature immediately after birth, suctioning of amniotic fluid at birth, tactile stimulation for resuscitation immediately after birth, use of continuous positive airway pressure for respiratory distress at term birth, respiratory monitoring in the delivery room, heart rate monitoring in the delivery room, and supraglottic airway use in neonates. The Task Force made treatment recommendations for each of the above topics after weighing evidence and discussion. In some cases, good practice statements have been provided for topics thought to be of particular interest to the resuscitation community when the evidence is insufficient to support a recommendation. Good practice statements are not recommendations but represent expert opinion. In order to facilitate the readers to understand the treatment recommendation well, in the recommendation basis part, the basic principle is briefly described. In addition, the existing problems and future research directions of each topic after the systematic reviews are also clearly stated.
Objective To explore the characters and therapy of the triple-negative breast cancer (TNBC). Methods The pertinent literatures with key words “triple-negative breast cancer”,“diagnosis”,and “therapy” were retrieved and reviewed. Results TNBC was a subtype of breast cancer characterized by negative expressions of estrogen receptor (ER),progesterone receptor (PR),and human epidermal growth factor receptor-2 (HER-2). The clinicopathologic feature and prognosis of TNBC were distinct from other breast cancer. The age of onset was younger,disease free survival and total survival rate were lower in the patients with TNBC. At present, the therapy guideline was lack of TNBC,which referred to the non-TNBC,including local surgery,systemic chemotherapy,and the target therapy was at the clinical trial stage. Conclusions TNBC is one of heterogeneity characteristics for the breast cancer,which has extra much difference. For improving the TNBC prognosis,we hope that more and more significant markers to TNBC in the future are found,which are useful to make individuation treatment.
